血液透析联合血液灌流治疗对慢性肾衰竭患者钙磷代谢及肾功能的影响

摘　　要：目的探讨血液透析(HD)联合血液灌流(HP)治疗对慢性肾衰竭(CRF)患者钙磷代谢及肾功能的影响。方法回顾性选取2020年1月—2023年1月济南市第三人民医院收治的80例CRF患者的病例资料,按治疗方法不同将其分为对照组和观察组,各40例。对照组采用HD治疗,观察组在对照组基础上联合HP治疗,两组均连续治疗12周。比较两组患者的临床疗效、钙磷代谢、肾功能及不良反应发生情况。结果观察组治疗总有效率为90.00%,高于对照组的72.50%,差异有统计学意义(P<0.05)。治疗后,观察组血钙水平高于对照组,血磷水平低于对照组,组间差异有统计学意义(P<0.05);观察组血肌酐、尿素氮、β2-微球蛋白及4 h尿蛋白定量水平均低于对照组,组间差异有统计学意义(P<0.05)。观察组不良反应发生率为2.50%,低于对照组的20.00%,差异有统计学意义(P<0.05)。结论采用HD联合HP治疗CRF患者的效果显著,可有效改善患者钙磷异常及肾功能,并降低不良反应发生率。Objective To investigate the effects of hemodialysis(HD)combined with hemoperfusion(HP)on calcium and phosphorus metabolism and renal function in patients with chronic renal failure(CRF).Methods The case data of 80 patients with CRF treated in Jinan Third People’s Hospital from January 2020 to January 2023 were retrospectively selected and divided into a control group and an observation group according to different treatment methods,with 40 cases in each group,the control group was treated with HD,and the observation group was treated with HP on the basis of the control group,both groups were treated continuously for 12 weeks.The clinical efficacy,calcium and phosphorus metabolism,renal function and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 90.00%,which was higher than 72.50%of the control group,the difference was statistically significant(P<0.05).After treatment,the blood calcium level in the observation group was higher than that in the control group,and the blood phosphorus level was lower than that in the control group,and the differences between the groups were statistically significant(P<0.05);the serum creatinine,urea nitrogen,β2-microglobulin and 4-hours urinary protein quantitative levels in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The incidence of adverse reactions in observation group was 2.50%,which was lower than 20.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion HD combined with HP is effective in the treatment of CRF patients,which can effectively improve calcium and phosphorus abnormalities and renal function,and reduce the incidence of adverse reactions.
