摘　　要：目的探讨收缩压心率乘积(RPP)联合Gensini评分对冠心病患者冠状动脉狭窄程度的预测价值。方法选取新乡医学院第一附属医院2023年1月—2024年1月收治的60例冠心病患者为研究对象。所有患者均行冠状动脉造影检查,计算Gensini评分,同时测量患者的收缩压(SBP)和心率(HR),计算RPP值。通过计算受试者工作特征(ROC)曲线下面积(AUC)评估RPP和Gensini评分对冠心病患者冠状动脉狭窄程度的预测能力。结果轻中度狭窄组与重度狭窄组三酰甘油水平、脑钠肽水平、肌红蛋白水平、D-二聚体水平、同型半胱氨酸水平、RPP、Gensini评分对比,组间差异有统计学意义(P<0.05)。Logistic回归分析结果显示,RPP值是影响冠心病患者冠状动脉狭窄程度的因素(P<0.05)。Pearson相关分析结果显示,RPP值与Gensini评分呈负相关(P<0.05)。ROC曲线结果显示,RPP值、Gensini评分及两项联合预测不同冠状动脉狭窄程度患者的AUC分别为0.954、0.894、0.982。结论RPP联合Gensini评分对冠心病患者冠状动脉狭窄程度具有较高的预测价值,可为临床诊断和治疗提供重要参考。Objective To explore the predictive value of systolic blood pressure heart rate product(RPP)combined with Gensini score for the degree of coronary stenosis in patients with coronary heart disease.Methods Sixty patients with coronary heart disease admitted to the First Affiliated Hospital of Xinxiang Medical College from January 2023 to January 2024 were selected as the research subjects.All patients underwent coronary angiography and Gensini score was calculated.Systolic blood pressure(SBP)and heart rate(HR)were also measured,and RPP values were calculated.Evaluate the predictive ability of RPP and Gensini score for the degree of coronary stenosis in patients with coronary heart disease by calculating the area under the receiver operating characteristic(ROC)curve(AUC).Results The comparison of triglyceride levels,brain natriuretic peptide levels,myoglobin levels,D-dimer levels,homocysteine levels,RPP,and Gensini scores between the mild to moderate stenosis group and the severe stenosis group showed statistically significant differences(P<0.05).The results of logistic regression analysis showed that RPP value is a factor affecting the degree of coronary artery stenosis in patients with coronary heart disease(P<0.05).The Pearson correlation analysis results showed a negative correlation between RPP value and Gensini score(P<0.05).The ROC curve results showed that the RPP value,Gensini score,and combined prediction of the area under the curve for patients with different degrees of coronary stenosis were 0.954,0.894,and 0.982,respectively.Conclusion The combination of RPP and Gensini score has high predictive value for the degree of coronary stenosis in patients with coronary heart disease,and can provide important references for clinical diagnosis and treatment.
