外剥内扎术基础上进行分层手术治疗复杂混合痔患者的临床效果

摘　　要：目的分析外剥内扎术基础上进行分层手术在复杂混合痔患者中的应用效果。方法选取2021年7月—2023年7月习水县妇幼保健院收治的152例复杂混合痔患者为研究对象,按照随机数字表法将其分为对照组与观察组,各76例。对照组采用外剥内扎术治疗,观察组在外剥内扎术基础上进行分层手术治疗。比较两组患者的术后恢复情况、术后疼痛及水肿程度、肛门功能、术后并发症发生情况。结果观察组伤口愈合时间、首次排便时间、住院时间均短于对照组,排便时疼痛视觉模拟(VAS)评分低于对照组,组间差异有统计学意义(P<0.05)。术后3、7、14 d,观察组VAS评分、水肿评分均低于对照组,组间差异有统计学意义(P<0.05)。术后1个月,观察组肛管静息压低于对照组,肛管舒张压高于对照组,组间差异有统计学意义(P<0.05)。观察组术后并发症发生率为7.89%,低于对照组的19.74%,差异有统计学意义(P<0.05)。结论在外剥内扎术基础上进行分层手术治疗复杂混合痔患者,具有更优异的临床效果,能减少患者的术后恢复时间,降低患者术后疼痛、水肿程度及并发症发生率,促进患者肛门功能的恢复。Objective To analyze the application effect of layered surgery on the basis of external stripping and internal ligation in patients with complex mixed hemorrhoids.Methods A total of 152 patients with complex mixed hemorrhoids admitted to Xishui County Maternal and Child Health Hospital from July 2021 to July 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 76 cases in each group.The control group was treated with external stripping and internal ligation,and the observation group was treated with layered surgery on the basis of external stripping and internal ligation.The postoperative recovery,postoperative pain and edema degree,anal function and postoperative complications were compared between the two groups.Results The wound healing time,first defecation time and hospital stay in the observation group were shorter than those in the control group,and the pain Visual Analogue Scale(VAS)score during defecation was lower than that in the control group,the differences between the two groups were statistically significant(P<0.05).At 3,7 and 14 days after operation,VAS scores and edema scores in the observation group were lower than those in the control group,the differences between the two groups were statistically significant(P<0.05).One month after operation,the anal canal resting pressure in the observation group was lower than that in the control group,and the anal canal diastolic pressure was higher than that in the control group,and the differences between the two groups were statistically significant(P<0.05).The incidence of postoperative complications in the observation group was 7.89%,which was lower than 19.74%in the control group,and the difference was statistically significant(P<0.05).Conclusion Layered surgery on the basis of external stripping and internal ligation has better clinical results in the treatment of complex mixed hemorrhoids,which can reduce the postoperative recovery time of patients,redu
