空气压力波治疗仪联合低分子肝素钙预防腹腔镜全子宫切除术后深静脉血栓形成的效果研究

摘　　要：目的探讨在腹腔镜全子宫切除手术的临床应用中,低分子肝素钙联合空气压力波治疗仪对术后深静脉血栓(DVT)形成的预防效果。方法选取2023年6月—2024年6月武威市人民医院收治的80例子宫肌瘤患者为研究对象,按照随机信封法将其分为对照组(n=34)和研究组(n=46)。两组均实施腹腔镜全子宫切除手术,在此基础上,对照组术后采用空气压力波治疗仪进行DVT预防干预,研究组术前和术后联用低分子肝素钙预防干预。比较两组患者的凝血功能、下肢血液循环状态及术后下肢DVT发生情况。结果术后7 d,研究组活化部分凝血活酶时间、凝血酶时间、凝血酶原时间均长于对照组,纤维蛋白原、D-二聚体均低于对照组,组间差异有统计学意义(P<0.05);研究组股静脉、腘静脉血流速度均快于对照组,大腿周径差小于对照组,组间差异有统计学意义(P<0.05);研究组术后下肢DVT发生率为4.35%,低于对照组的20.59%,差异有统计学意义(P<0.05)。结论在子宫肌瘤腹腔镜全子宫切除术后DVT的预防治疗中,采用低分子肝素钙联合空气压力波治疗仪进行预防干预,能够进一步调节凝血因子活性,保证下肢血液循环正常,促进术后体征恢复,治疗实用性较高。Objective To investigate the preventive effect of low molecular weight heparin calcium combined with air pressure wave therapy instrument on postoperative Deep Vein Thrombosis(DVT)in the clinical application of laparoscopic total hysterectomy.Methods Eighty patients with uterine fibroids admitted to Wuwei People's Hospital from June 2023 to June 2024 were selected as the research objects,and were divided into a control group(n=34)and a research group(n=46)according to random envelope method.Laparoscopic total hysterectomy was performed in both groups,on this basis,the control group received postoperative DVT preventive intervention with air pressure wave therapy,and the research group received preoperative and postoperative low molecular weight heparin calcium preventive intervention.The coagulation function,lower limb blood circulation status and postoperative DVT occurrence of lower limbs of the two groups were compared.Results 7 days after surgery,the activated partial thromboplastin time,thrombin time and prothrombin time in the research group were longer than those in the control group,while the fibrinogen and D-dimer were lower than those in the control group,the differences between groups were statistically significant(P<0.05);the blood flow velocity of femoral vein and popliteal vein in the research group were faster than those in the control group,and the difference of thigh circumference was smaller than that in the control group,the differences between groups were statistically significant(P<0.05);the incidence of DVT was 4.35%in the research group,which was lower than 20.59%in the control group,and the difference was statistically significant(P<0.05).Conclusion In the prevention and treatment of DVT after laparoscopic total hysterectomy for uterine fibroids,the use of low molecular weight heparin calcium combined with air pressure wave therapy instrument for preventive intervention can further regulate the activity of coagulation factors,ensure the normal blood circulation of lower limbs,promote the r
