龙胆泻肝汤加减联合更昔洛韦治疗带状疱疹患者的临床效果

摘　　要：目的探讨龙胆泻肝汤加减联合更昔洛韦治疗带状疱疹患者的临床效果。方法选取2020年2月—2023年2月莒南县中医医院收治的84例带状疱疹患者为研究对象,按随机数字表法将其分为对照组和观察组,各42例。对照组采用更昔洛韦治疗,观察组在对照组基础上联合龙胆泻肝汤加减治疗,两组均持续治疗4周。比较两组的临床疗效、疼痛程度、血清疼痛介质、血清炎性因子及不良反应发生情况。结果观察组治疗总有效率为97.62%,高于对照组的71.42%,差异有统计学意义(P<0.05)。T3、T4时,观察组疼痛视觉模拟评分均低于对照组,组间差异有统计学意义(P<0.05);治疗4周后,观察组血清P物质、前列腺素E2、肿瘤坏死因子-α及白细胞介素-6水平均低于对照组,组间差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论龙胆泻肝汤加减方联合更昔洛韦治疗带状疱疹患者可有效提高临床疗效,调节血清疼痛介质及炎性因子水平,减轻患者疼痛程度。Objective To investigate the clinical effect of modified Longdan Xeigan decoction combined with ganciclovir in the treatment of herpes zoster.Methods A total of 84 patients with herpes zoster admitted to Junan County Hospital of Traditional Chinese Medicine from February 2020 to February 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 42 cases in each group.The control group was treated with ganciclovir,and the observation group was treated with modified Longdan Xeigan decoction on the basis of the control group,both groups were treated continuously for 4 weeks.The clinical effect,pain degree,serum pain mediators,serum inflammatory factors and occurrence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 97.62%,which was higher than 71.42%of the control group,the difference was statistically significant(P<0.05).At T3 and T4,the pain Visual Analogue Scale score of the observation group were lower than those of the control group,and the differences between the two groups were statistically significant(P<0.05);after 4 weeks of treatment,the serum levels of substance P,prostaglandin E2,tumor necrosis factor-αand interleukin-6 in observation group were lower than those in the control group,and the differences between the two groups were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Modified Longdan Xeigan decoction combined with ganciclovir can effectively improve clinical efficacy,regulate the levels of serum pain mediators and inflammatory factors,and reduce the degree of pain in patients with herpes zoster.
