摘　　要：目的探究罗哌卡因(Rop)复合地佐辛腹横肌平面阻滞(TAP)结合静脉全身麻醉(GA)对妇科腹腔镜手术患者术后疼痛控制及康复的影响。方法共纳入本院收治的70例妇科腹腔镜手术患者,时间为2022年3月-2024年2月,以随机数表法分为两组,即对照组(n=35)、研究组(n=35),对照组静脉GA后予以Rop行TAP麻醉,研究组静脉GA后予以Rop复合地佐辛TAP麻醉技术;于术后12h、24h、48h、72h时,比较两组疼痛程度[视觉模拟评分法(VAS)];比较两组术后康复指标、不良反应发生率、患者麻醉满意度。结果与对照组比较,研究组术后各时点VAS评分更低(P<0.05);与对照组比较,研究组术后排气时间、下床时间及住院时间均更短,住院费用更低(P<0.05);与对照组比较,研究组术后不良反应发生率更低,患者对镇痛满意度评分更高(P<0.05)。结论Rop复合地佐辛TAP结合GA应用于妇科腹腔镜手术患者中,可提高术后疼痛控制效果,促进患者术后康复,降低不良反应发生率,提高患者满意度。Objective To investigate the effects of ropivacaine(Rop)combined with dexmedetomidine transversus abdominis plane block(TAP)and intravenous general anesthesia(GA)on postoperative pain control and rehabilitation in patients undergoing gynecological laparoscopic surgery.Methods A total of 70 gynecological laparoscopic surgery patients admitted to our hospital from March 2022 to February 2024 were randomly divided into two groups using a random number table method:a control group(n=35)and a study group(n=35).The control group received Rop and TAP anesthesia after intravenous GA,while the study group received Rop combined with Dexmedetomidine and TAP anesthesia after intravenous GA;compare the pain levels between the two groups at 12,24,48,and 72 hours postoperatively using the Visual Analog Scale(VAS);compare the postoperative rehabilitation indicators,incidence of adverse reactions,and patient anesthesia satisfaction between two groups.Results Compared with the control group,the VAS scores of the study group were lower at all postoperative time points(P<0.05);compared with the control group,the study group had shorter postoperative exhaust time,ambulation time,and hospitalization time,and lower hospitalization costs(P<0.05);compared with the control group,the adverse reaction incidence rate was lower in the study group,and patients had higher satisfaction scores with analgesia(P<0.05).Conclusion The application of Rop combined with Dexmedetomidine TAP and GA in gynecological laparoscopic surgery patients can improve postoperative pain control,promote postoperative recovery,reduce the incidence of nausea and vomiting,and improve patient satisfaction.
