双歧杆菌乳杆菌三联活菌片联合孟鲁司特钠对咳嗽变异性哮喘儿童炎症反应及TIMP-1、CysLTs水平的影响

摘　　要：目的分析双歧杆菌乳杆菌三联活菌片联合孟鲁司特钠对咳嗽变异性哮喘(CVA)儿童炎症反应及血清基质金属蛋白酶组织抑制剂-1(TIMP-1)、半胱氨酰白三烯(CysLTs)水平的影响。方法回顾性分析太原妇幼保健院2022年4月—2024年5月收治的97例CVA患儿的病历资料,以治疗方法将其分成对照组(48例,接受孟鲁司特钠治疗)和观察组(49例,接受双歧杆菌乳杆菌三联活菌片联合孟鲁司特钠治疗)。两组均治疗3个月。比较两组临床疗效、炎症反应指标、气道重塑指标、血清学指标、肺功能指标及不良反应。结果观察组治疗总有效率高于对照组(P<0.05)。观察组患儿治疗前后巨噬细胞炎性蛋白-1α(MIP-1α)、γ-干扰素(IFN-γ)、嗜酸性粒细胞趋化因子(Eotaxin)差值均高于对照组(P<0.05)。观察组患儿治疗前后气道壁厚度、气道横截面积、气道壁面积差值均高于对照组(P<0.05)。观察组患儿治疗前后TIMP-1、CysLTs差值均高于对照组(P<0.05)。观察组患儿治疗前后最大呼气峰流速(PEF)、第1秒用力呼气容积(FEV1)、用力肺活量(FVC)差值均高于对照组(P<0.05)。两组不良反应总发生率的比较,差异无统计学意义(P>0.05)。结论双歧杆菌乳杆菌三联活菌片联合孟鲁司特钠治疗CVA患儿疗效确切,可改善患儿肺功能,减轻炎症反应,逆转气道重塑,调节血清TIMP-1、CysLTs表达,且安全可靠。Objective To investigate the effects of lactobacillus bifidobacterium triactive tablets combined with Montelukast sodium on inflammatory response and serum tissue inhibitor of matrix metalloproteinase-1(TIMP-1)and cysteinotriene(CysLTs)levels in children with cough variant asthma(CVA).Methods The medical records of 97 cases of CVA children admitted to Taiyuan Maternal and Child Health Hospital from April 2022 to May 2024 were retrospectively analyzed,and they were divided into control group(48 cases,received Montelukast sodium)and observation group(49 cases,received lactobacillus bifidobacterium triactive tablets combined with Montelukast sodium)by treatment method.Both groups were treated for 3 months.The clinical efficacy,inflammatory response,airway remodeling,serological indexes,lung function and adverse reactions were compared in 2 groups.Results The observation group with total effective rate was higher than control group(P<0.05).The observation group with difference of macrophage inflammatory protein-1α(MIP-1α),γ-interferon(IFN-γ)and eosinophilic chemotactic factor(Eotaxin)before and after treatment were higher than control group(P<0.05).The observation group with differences of airway wall thickness,airway cross-sectional area and airway wall area before and after treatment were higher than control group(P<0.05).The difference of TIMP-1 and CysLTs before and after treatment in observation group was higher than control group(P<0.05).The observation group with difference of maximum expiratory peak flow rate(PEF),forced expiratory volume in the first second(FEV1)and forced vital capacity(FVC)before and after treatment were higher than control group(P<0.05).There was no difference in total adverse reactions incidence in 2 groups(P>0.05).Conclusion Children with CVA were treated with bifidobacterium Lactobacillus triple viable tablets combined with Montelukast sodium,which can improve lung function,reduce inflammation,reverse airway remodeling,regulate serum TIMP-1 and CysLTs expression,and is safe and rel
