一例猫传染性腹膜炎的诊断

摘  要:1岁布偶公猫,就诊前食欲废绝,精神沉郁,腹腔肿大且触诊有波动感,疑似发生猫传染性腹膜炎(feline infectious peritonitis,FIP)。为进行确诊,笔者首先对患猫进行血液学检查[血生化检查、血常规检查和血清淀粉样蛋白(serum amyloid A,SAA)质量浓度检测]和影像学检查,然后取患猫腹腔腹水,提取病毒DNA/RNA后采用RT-PCR法对猫冠状病毒(Feline coronavirus,FCoV)N基因进行扩增和测序,最后基于测序结果进行遗传进化分析。结果表明:患猫血液中球蛋白升高,提示肝脏损伤或出现炎症感染;白蛋白与球蛋白比值(白球比)为0.34(<0.40),提示患猫肾功能异常、体内存在炎症反应;总蛋白升高,提示可能发生脱水或炎症反应,或肝脏、肾脏器官损伤及功能不全;淀粉酶含量升高,提示可能存在胰脏、肾脏损伤及功能不全,以及肠管内炎症。患猫白细胞数目、中性粒细胞数目和中性粒细胞百分比偏高,淋巴细胞百分比偏低,血细胞比容>30%,但均在参考值范围内。SAA质量浓度为0.173μg/mL,在参考值范围(0~5.000μg/mL)内。患猫左肾和右肾的长直径分别为3.5 cm和3.8 cm,右肾较左肾稍大,两肾的皮髓质界限清晰,均发现有轻微髓质环征,且存在脓性肉芽肿病变;腹膜广泛性回声升高,可见大量游离性液体,腹腔内淋巴结回声均匀且存在大量积液;空肠淋巴结肿大。从患猫腹腔腹水中扩增出大小为1134 bp的FCoV N基因(命名为FJFZ202201),该基因与中国毒株FCoV strain HLJ HRB 201613和FCoV strain HLJ HRB 201611 N基因的相似性最高,为94.0%,且处于FCoV-Ⅰ型分支中。说明患猫由于感染了FCoV-Ⅰ型毒株而发生渗出性FIP。A 1-year-old male Ragdoll cat showed loss of appetite,depression,abdominal cavity,and fluctuating sensation on palpation before the visit,which was suspected to be feline infectious peritonitis(FIP).In order to confirm the diagnosis,the author first performed blood tests[blood biochemical test,routine blood test,and serum amyloid A(SAA)mass concentration testing]and imaging examination on the affected cat.Then the fetal ascites were taken;after the viral DNA/RNA was extracted,the Feline coronavirus(FCoV)N gene was amplified and sequenced by RT-PCR.Finally,genetic evolution analysis was carried out based on the sequencing results.The results showed that elevated blood globulin in the affected cat indicated liver damage or inflammatory infection;the albumin-to-globulin ratio(white globulin ratio)was 0.34(<0.40),indicating abnormal kidney function and inflammatory response in the cat;elevated total protein suggested suffering from dehydration or inflammation,or liver and kidney organ damage and dysfunction.Elevated amylase content suggested pancreatic or renal damage or insufficiency,as well as inflammation of the bowel.The number of white blood cells,neutrophils and neutrophil percentage were high,the percentage of lymphocytes was low,and the hematopoietic volume(HCT)was>30%,but all were within the reference value range.The concentration of SAA was 0.173μg/mL,which was within the reference value range(0-5.000μg/mL).The left and right kidneys diameters were 3.5 cm and 3.8 cm,respectively,and the right kidney was slightly larger than the left kidney.The cortical medullary boundaries of both kidneys were clear,and slight medullary rings were found,and purulent granulomatous lesions were present;elevated peritoneal generalized echogenicity was increased,with large amounts of free fluid,uniform intraperitoneal lymph node echogenicity,and large effusions and swollen jejunal lymph nodes.The FCoV N gene(named FJFZ202201)with a size of 1134 bp was amplified from the ascites fluid of the affected cat,which had the highest
