摘　　要：皮肤诺卡菌病是由诺卡菌属通过皮肤伤口引发的感染,其中包括巴西诺卡菌和星形诺卡菌等多种诺卡菌。皮肤诺卡菌病通常表现为皮下结节和瘘管,症状不具特异性。对于皮肤诺卡菌病的治疗,及时和准确的诊断具有至关重要的作用。尽管磺胺类药物依然是治疗皮肤诺卡菌病的首选药物,但随着耐药性菌株和多重耐药性菌株的出现,为临床治疗带来了新的挑战。确立精确的体外药物敏感性谱并选择对其敏感的抗生素,对于临床药物使用具有重要的指导作用。以1例皮肤巴西诺卡菌病患者为例,从病案回顾及诊疗经历进行分析,以期为临床类似疾病的诊治工作提供参考。Cutaneous nocardiosis is an infection caused by the genus Nocardia through skin wounds,including various Nocardia species such as Nocardia brasiliensis and Nocardia asteroides.Cutaneous nocardiosis usually presents as subcutaneous nodules and fistulas,and the symptoms are not specific.Timely and accurate diagnosis plays a crucial role in the treatment of cutaneous nocardiosis.Although sulfonamides remain the first choice for the treatment of cutaneous nocardiosis,the emergence of drug-resistant strains and multi-drug resistant strains has brought new challenges to clinical treatment.Establishing an accurate in vitro drug susceptibility profile and selecting sensitive antibiotics have important guiding significance for the clinical use of drugs.Taking a patient with cutaneous nocardiosis caused by Nocardia brasiliensis as an example,this article analyzes the medical record review and the diagnosis and treatment process,aiming to provide a reference for the diagnosis and treatment of similar diseases in clinical practice.
