五皮五藤饮辅助治疗风湿热痹型膝骨关节炎临床研究

摘　　要：目的探讨五皮五藤饮辅助治疗风湿热痹型膝骨关节炎(knee osteoarthritis,KOA)的临床疗效。方法选取2021年5月—2023年5月本院收治的78例风湿热痹型KOA患者,采用随机数字表法分为对照组和观察组,每组39例。对照组给予双氯芬酸钠口服、玻璃酸钠膝关节腔内注射治疗,观察组在对照组基础上给予五皮五藤饮辅助治疗。比较2组患者临床疗效及不良反应发生情况,比较2组患者治疗前后中医证候积分及血清、关节液肿瘤坏死因子(tumor necrosis factor,TNF)-α、白细胞介素(interleukin,IL)-6、IL-1水平。结果治疗后,观察组总有效率显著高于对照组(P<0.05)。观察组各项中医证候积分及血清、关节液TNF-α、IL-6、IL-1水平显著低于对照组(P<0.05)。治疗期间,观察组不良反应发生率显著低于对照组(P<0.05)。结论五皮五藤饮辅助治疗风湿热痹型KOA可显著改善患者临床症状,降低血清及关节液中炎症因子表达,临床疗效确切且安全性较高。Objective To explore the clinical efficacy of Wupi Wuteng decoction in the adjuvant treatment of knee osteoarthritis(KOA)with wind-damp-heat bi type.Methods Seventy-eight KOA patients with wind-damp-heat bi type admitted to our hospital from May 2021 to May 2023 were selected and divided into control group and observation group by random number table method,with 39 cases in each group.The control group was treated with oral diclofenac sodium and intraarticular injection of sodium hyaluronate in the knee joint,while the observation group was treated with adjuvant treatment with Wupi Wuteng decoction on the basis of the control group.The clinical efficacy and adverse reactions of the two groups of patients were compared.The TCM syndrome scores,serum and joint fluid tumor necrosis factor(TNF)-α,interleukin(IL)-6,and IL-1 levels before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The scores of various TCM syndromes and the levels of TNF-α,IL-6,and IL-1 in serum and joint fluid in the observation group were significantly lower than those in the control group(P<0.05).During treatment,the incidence of adverse reactions in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Wupi Wuteng decoction in the adjuvant treatment of knee osteoarthritis(KOA)with wind-damp-heat bi type could significantly improve the clinical symptoms of patients,reduce the expression of inflammatory factors in serum and joint fluid,with definite clinical efficacy and high safety.
