摘　　要：目的探索PDCA循环管理法促进人血白蛋白的临床合理使用。方法通过PDCA循环管理法,确定该院人血白蛋白管理目标,制定针对性的整改措施,比较干预前(2019年1-12月,)与干预后(第一阶段2020年1-12月、第二阶段2021年1-12月、第三阶段2022年1-12月、第四阶段2023年1-12月)人血白蛋白临床使用情况和医嘱合理性。结果人血白蛋白的使用率由干预前的13.8%降至干预后的11.5%(P<0.05),人血白蛋白人均使用量由干预前的121.49 g降至118.42 g,下降了2.53%.人血白蛋白住院医嘱合理率由干预前的76.39%提升至持续改进后的99.44%(P<0.05),人血白蛋白人均使用疗程由干预前的9.82 d降至6.40 d,下降了34.83%.结论通过多学科协作结合PDCA循环管理法可实现人血白蛋白使用率、人均使用量、使用疗程有效下降,医嘱合理率持续提升。基本实现拟定目标:提高人血白蛋白的临床合理应用,节约医药资源。Objective To explore the effectiveness of the Plan-Do-Check-Act(PDCA)cycle management method in promoting rational clinical use of human albumin.Methods The PDCA cycle management method was used to establish the human albumin management objectives in hospitals,and targeted rectification measures were formulated.Clinical use and rationality of medical orders for human albumin were compared before intervention(January-December,2019)and after intervention(January-December,2020 in the first stage,January-December,2021 in the second stage,January-December,2022 in the third stage and January-December,2023 in the fourth stage).Results The utilization rate of human albumin decreased from 13.8% before intervention to 11.5% after intervention(P<0.05),and the average use of human albumin decreased from 121.49g to 118.42g(a drop of 2.53%).The rationality rate of inpatient medical orders for human albumin increased from 76.39% before intervention to 99.44% after continuous improvemen(t P<0.05),and the average treatment course ofhuman albumin use decreased from 9.82 days to 6.40 days(a drop of 34.83%).Conclusion Through the multidisciplinary cooperation combined with PDCA cycle management method,the utilization rate,average use and treatment course of human albumin use can be effectively reduced,and the rationality rate of medical orders can be continuously improved,generally reaching the intended targets(improve the rational clinical application of human albumin and save medical resources).
