益气活血法治疗糖尿病肾病气虚血瘀证临床观察

摘　　要：目的观察益气活血法治疗糖尿病肾病(DKD)气虚血瘀证的临床效果。方法选取符合纳入标准的患者280例,随机分为治疗组和对照组,各140例。对照组用西医常规治疗,治疗组用常规治疗结合归芪饮治疗,观察8周。比较两组治疗前后中医证候积分、肾小球滤过率(eGFR)、胱抑素C(Cys-C)、肌酐(Scr)、尿白蛋白肌酐比(ACR)、三磷酸尿苷(U-TP)水平变化,中医证候总有效率及临床综合总有效率。结果治疗后,两组eGFR水平较治疗前升高,Scr、Cys-C、ACR、U-TP水平较治疗前降低,且治疗组改善明显优于对照组(P<0.05)。治疗后,两组中医证候积分均下降,且治疗组明显低于对照组(P<0.05)。治疗组中医证候总有效率为92.86%(130/140),高于对照组的77.14%(108/140)(P<0.05)。治疗组临床综合总有效率为94.29%(132/140),明显高于对照组的79.29%(111/140)(P<0.05)。结论益气活血法可以降低DKD患者的中医证候积分和尿蛋白水平、改善肾功能、提高临床疗效。Objective To observe the clinical effect of benefiting qi for activating blood circulation method in the treatment of diabetic kidney disease(DKD)of qi deficiency and blood stasis syndrome.Methods A total of 280 patients met the criteria were selected and randomly divided into treatment group and control group,with 140 cases in each group.The control group was treated with basic Western medicine,and the treatment group was treated with basic Western medicine and Guiqi drink.After 8 weeks,TCM syndrome scores,glomerular filtration rate(eGFR),cystatin C(Cys-C),creatinine(Scr),urinary albumin creatinine ratio(ACR)and uridine triphosphate(U-TP)levels were compared between the two groups before and after treatment.Results After treatment,eGFR levels in both groups were higher than before treatment,Scr,Cys-C,ACR and U-TP levels were lower than before treatment,and the improvement in the treatment group was significantly better than that in the control group(P<0.05).After treatment,the TCM syndrome scores of both groups decreased,and the treatment group was significantly lower than the control group(P<0.05).The total effective rate of TCM syndromes in the treatment group was 92.86%(130/140),which was higher than that in the control group(77.14%,108/140)(P<0.05).The total effective rate of the treatment group was 94.29%(132/140),which was significantly higher than that of the control group(79.29%,111/140)(P<0.05).Conclusion Guiqi drink can reduce TCM syndrome score and urinary protein level of DKD patients,improve renal function,and improve clinical effect.
