择时耳穴压豆+太极拳左右揽雀尾在脱出型腰椎间盘突出症患者中的应用

摘　　要：目的 探究择时耳穴压豆+太极拳左右揽雀尾在脱出型腰椎间盘突出症(LDH)患者术后康复期中的应用效果。方法 前瞻性选取2023年1月至12月宝鸡市中医医院脊柱中心4病区收治的168例脱出型LDH患者,按随机数字表法分为对照组与研究组各84例。研究组男51例,女33例,年龄(58.79±6.53)岁,体重指数(23.52±2.24)kg/m^(2),病程(4.79±1.53)年,突出节段:L_(3~4)26例、L_(4~5)31例、L_(5)~S_(1)27例。对照组男54例,女30例,年龄(59.21±6.44)岁,体重指数(23.43±2.35)kg/m^(2),病程(4.61±1.44)年,突出节段:L_(3~4)29例、L_(4~5)32例、L_(5)~S_(1)23例。两组均行经皮椎间孔入路内窥镜下椎间盘切除术,对照组术后实施常规护理,研究组在对照组基础上实施择时耳穴压豆+太极拳左右揽雀尾干预,两组患者均护理12周。比较两组不同时间点疼痛程度[数字评分法(NRS)评分]、腰部功能[Oswestry功能障碍指数(ODI)]、生活质量[生活质量综合评定问卷(GQOLI-74)]。采用独立样本t检验、χ^(2)检验进行统计学分析。结果 研究组术后第6周、第12周的NRS评分均低于对照组[(4.04±0.86)分比(5.67±1.18)分、(1.71±0.60)分比(2.94±0.91)分](均P<0.05);ODI评分均低于对照组[(15.92±2.40)分比(21.71±3.15)分、(7.20±1.43)分比(13.61±2.19)分](均P<0.05);心理功能、躯体功能、社会功能、物质生活状态评分均高于对照组(均P<0.05)。结论 择时耳穴压豆+太极拳左右揽雀尾能有效缓解脱出型LDH患者术后康复期疼痛程度,改善患者腰部功能及生活质量。Objective To explore the application effect of elective ear acupoint pressing bean+Taijiquan round the tail of the bird in the postoperative rehabilitation period of prolapse lumbar disc herniation(LDH)patients.Methods A total of 168 patients with prolapse LDH from January to December 2023 in Ward 4,Spine Center,Baoji Hospital of Traditional Chinese Medicine were prospectively selected and were divided into a control group and a study group with 84 cases in each group according to the random number table method.In the study group,there were 51 males and 33 females,aged(58.79±6.53)years,with a body mass index of(23.52±2.24)kg/m^(2) and a disease course of(4.79±1.53)years,protruding segments:L_(3~4) in 26 cases,L_(4~5) in 31 cases,and L_(5)~S_(1) in 27 cases.In the control group,there were 54 males and 30 females,aged(59.21±6.44)years,with a body mass index of(23.43±2.35)kg/m^(2) and a course of disease of(4.61±1.44)years,protruding segments:L_(3~4) in 29 cases,L_(4~5) in 32 cases,and L_(5)~S_(1) in 23 cases.Both groups underwent endoscopic discectomy through percutaneous foraminal approach,the control group received regular care after surgery,and the study group received elective ear acupoint pressing bean+Taijiquan round the tail of the bird for 12 weeks.The pain degree[Numeric Rating Scale(NRS)score],lumbar function[Oswestry Disability Index(ODI)],and quality of life[Generic Quality of Life Inventory-74(GQOLI-74)]were compared between the two groups at different time points.Independent sample t test andχ^(2) test were used for statistical analysis.Results The NRS scores of the study group 6 and 12 weeks after surgery were lower than those of the control group[(4.04±0.86)points vs.(5.67±1.18)points,(1.71±0.60)points vs.(2.94±0.91)points](both P<0.05);the ODI scores were lower than those of the control group[(15.92±2.40)points vs.(21.71±3.15)points,(7.20±1.43)points vs.(13.61±2.19)points](both P<0.05);the scores of psychological function,somatic function,social function,and material life status were
