多育节荚孢致特重度烧伤患者血流感染1例

摘　　要：多育节荚孢(Lomentospora prolificans)是一种毒力大、侵袭力强、耐药性高的丝状真菌,常引起免疫功能低下患者播散性感染,治疗困难,具有高死亡率。本文报道一例因“煤气泄漏爆炸烧伤入院”的59岁特重度烧伤男性患者,住院期间多次进行清创和经验性抗感染治疗,但感染仍未控制,最终通过创面分泌物培养和血培养明确病原体为多育节荚孢,由于患者烧伤严重,并发症多,非医嘱离院一周后死亡。Lomentospora prolificans is a highly virulent,invasive,and multidrug-resistant filamentous fungus that frequently causes disseminated infections in immunocompromised patients.These infections are notoriously difficult to manage and are associated with high mortality rates.This case report describes a 59-year-old male patient who sustained extensive burns from a gas explosion.During his hospitalization,he underwent multiple debridements and received empirical antimicrobial therapy.Lomentospora prolificans was identified as the causative agent of uncontrolled infection through wound secretion and blood cultures.Despite intensive medical interventions,the patient succumbed to the severity of his burns and multiple complications one week after leaving the hospital against medical advice.
