疏肝利胆汤治疗急性结石性胆囊炎术后患者临床疗效观察

摘　　要：目的分析急性结石性胆囊炎术后患者应用疏肝利胆汤的治疗效果。方法选取77例急性结石性胆囊炎患者(苏州市中西医结合医院收治),分为对照组(38例,腹腔镜胆囊切除术+术后常规治疗)、研究组(39例,疏肝利胆汤+对照组),分组方法:随机数字表法,连续治疗4周。比较两组临床疗效(治疗后),围术期指标,炎性反应及淀粉酶水平、中医证候积分、消化功能指标变化(治疗前后)。结果治疗后,研究组总有效率高于对照组;研究组开始进食固体食物、抗生素使用、腹胀缓解、体温恢复、白细胞计数恢复正常、住院等时间均短于对照组;治疗后与治疗前比较,两组中医证候积分、血清血管活性肠肽(VIP)、细胞间黏附分子-1(ICAM-1)、髓过氧化物酶(MPO)、淀粉酶、基质金属蛋白酶-9(MMP-9)、生长抑素(SS)水平降低,且研究组更低(两组间治疗后相比);两组血清胃动素(MTL)水平升高,且两组间治疗后相比,研究组更高(P<0.05)。结论急性结石性胆囊炎术后患者应用疏肝利胆汤治疗,有助于缓解临床症状,优化围术期指标,改善消化功能,下调炎性因子指标及淀粉酶水平,提高临床疗效。Objective To analyze the therapeutic effect of Shugan Lidan Decoction(疏肝利胆汤)on postoperative patients with acute calculous cholecystitis.Methods Seventy-seven patients with acute calculous cholecystitis in Suzhou Hospital of Integrated Traditional Chinese and Western Medicine were selected.They were divided into the control group(38 cases)and the research group(39 cases).The grouping method was random number table method.Both groups were given laparoscopic cholecystectomy,while the control group was given the routine treatment after operation.The research group was given Shugan Lidan Decoction after operation on the basis of the control group,and the two groups were treated for 4 weeks continuously.The perioperative indexes,the clinical efficacy after treatment,the traditional Chinese medicine syndrome scores,inflammatory reaction,amylase level and digestive function indexes before and after treatment were compared between the two groups.Results After treatment,the total effective rate of the research group was higher than that of the control group.The time of using antibiotics,starting to eat solid food,hospitalization time,body temperature recovery time,abdominal distension relief time and white blood cell count recovery time of the research group were shorter than those in the control group.After treatment,compared with those before treatment,the traditional Chinese medicine syndrome scores,the levels of serum intercellular adhesion molecule-1(ICAM-1),myeloperoxidase(MPO),matrix metalloproteinase-9(MMP-9),vasoactive intestinal peptide(VIP),somatostatin(SS)and amylase of the two groups decreased,and the levels of the research group were even lower than those of the control group.The serum motilin(MTL)levels of the two groups increased,and the level in the research group was higher(P<0.05).Conclusion The application of Shugan Lidan Decoction in postoperative patients with acute calculous cholecystitis is helpful to relieve the clinical symptoms of patients,optimize the perioperative indicators,improve
