摘　　要：目的探究活血止痛膏联合推拿对气滞血瘀型急性腰扭伤患者临床症状、腰功能以及生活质量的影响。方法2021年1月—2023年1月期间,在医院随机选取气滞血瘀型急性腰扭伤患者98例,采用随机数字表法分为观察组51例和对照组47例。所有患者均行常规治疗,对照组在其基础上联合推拿治疗,观察组在对照组基础上联合活血止痛膏治疗。对比两组治疗疗效及安全性,治疗前后对患者进行中医证候评分、腰椎活动度(ROM)检测,同时采用腰痛评分标准(the oswestry disability index,ODI)、简明健康状况调查表(short form 36 health survey questionnaire,SF-36)对所有患者进行评估。结果观察组治疗总有效率高于对照组(90.20%vs 74.47%)(P<0.05),两组均未发生严重不良反应。治疗后,观察组中医证候“腰部活动受限、肌肉痉挛、肿胀以及腰痛”得分均低于对照组(P<0.05)。治疗后,观察组左右侧屈、前屈角度均大于对照组(P<0.05),两组后伸角度无差异(P>0.05)。治疗后,观察组ODI得分低于对照组,SF-36高于对照组,差异具有统计学意义(P<0.05)。结论活血止痛膏联合推拿治疗气滞血瘀型急性腰扭伤,能够有效缓解病症,改善患者腰功能障碍,提高患者的生活质量,且安全性较好。Objective To explore the effects of Huoxue Zhitong Ointment(活血止痛膏)combined with Tuina on clinical symptoms,lumbar function and quality of life of patients with acute lumbar sprain of Qi stagnation and blood stasis type.Methods From January 2021 to January 2023,98 patients with acute lumbar sprain of Qi stagnation and blood stasis type were randomly selected in the hospital and divided into observation group(51 cases)and control group(47 cases)by random number table method.All patients received routine treatment.The control group combined with Tuina treatment on the basis of the control group,and the observation group combined with Huoxue Zhitong Ointment on the basis of the control group.The therapeutic efficacy and safety of the two groups were compared.TCM syndrome score and lumbar motion range(ROM)were detected before and after treatment,and all patients were evaluated by low back pain score standard(ODI)and short form 36 health survey questionnaire(SF-36).Results The total effective rate of the observation group was higher than that of the control group(90.20%vs 74.47%)(P<0.05),and serious adverse reactions occurred in both groups.After treatment,the scores of traditional Chinese medicine symptoms“waist limitation,muscle spasm,swelling and back pain”in the observation group were lower than those in the control group(P<0.05).After treatment,the angles of lateral flexion and anterior flexion in the observation group were higher than those in the control group(P<0.05),and there was no difference in the angle of extension between the two groups(P>0.05).After treatment,ODI score in the observation group was lower than that of the control group,SF-36 score was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Huoxue Zhitong Ointment combined with Tuina to treat acute lumbar sprain of Qi stagnation and blood stasis type can effectively relieve the disease,improve the lumbar dysfunction and the quality of life of the patients,with good safety.
