摘　　要：目的探讨CO_(2)激光灸联合内异止痛方治疗子宫内膜异位症的临床疗效及对炎症因子、疼痛的影响。方法102例患者随机分为对照组和观察组各51例,对照组给予常规治疗和CO_(2)激光灸,观察组在对照组基础上加用内异止痛方,疗程4周。检测简化McGill问卷评分、血清TNF-α、IL-6、PGE_(2)、ICAM-1、VEGF、E_(2)、LH、FSH水平、中医证候积分、临床疗效。结果观察组总有效率高于对照组(P<0.05)。治疗后,观察组VAS、PPI评分低于对照组(P<0.05),PRI评分较前降低(P<0.05),TNF-α、IL-6、PGE_(2)、ICAM-1、VEGF、E_(2)、LH、FSH水平低于对照组(P<0.05),观察组小腹冷痛、月经推迟积分低于对照组(P<0.05),带下量多积分较前降低(P<0.05)。结论CO_(2)激光灸联合内异止痛方治疗子宫内膜异位症临床疗效显著,能降低患者炎症因子、性激素水平和疼痛评分,改善患者生活质量。Objective To explore the effect of CO_(2)laser moxibustion combined with Neiyi Zhitong Formula(内异止痛方)on inflammatory factors and pain degree in patients with endometriosis.Methods One hundred and two patients with endometriosis were randomly assigned into two groups,each with 51 cases.The control group was given conventional treatment plus CO_(2)laser moxibustion.Based on this,the observation group added Neiyi Zhitong Decoction.The treatment in both groups lasted for 4 weeks.Then the simplified McGill Pain Questionnaire including pain rating index(PRI),visual analoge scale(VAS)and present pain index(PPI),serum levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),prostaglandin E_(2)(PGE_(2)),intercellular adhesion molecule-1(ICAM-1)and vascular endothelial growth factor(VEGF),luteinizing hormone(LH),estradiol(E2),follicle stimulating hormone(FSH),TCM syndrome score and clinical efficacy were compared between two groups.Results The observation group reported a higher clinical efficacy than the control group(P<0.05).After treatment,the observation group scored lower on VAS and PPI compared to the control group,with statistical difference(P<0.05).A reduction in PRI was observed in all patients,with no statistical difference between two groups(P>0.05).The serum levels of TNF-α,IL-6,PGE_(2),ICAM-1,VEGF,E_(2),LH and FSH were lower in the observation group than those in the control group(P<0.05).TCM syndrome scores including menstrual pain and delayed menstruation cycle were remarkably lower in the observation group than those in the control group(P<0.05).The reduction in scores of excessive vaginal discharge was observed in all subjects after treatment(P<0.05).Conclusion CO_(2)laser moxibustion combined with Neiyi Zhitong Decoction in the treatment of endometriosis can attenuate inflammatory and sex hormone levels,alleviate the pain and improve the quality of life of patients.
