疗法改善脑卒中患者肢体功能障碍的效果

摘　　要：目的评价针灸联合康复疗法改善脑卒中患者肢体功能障碍的效果。方法2020年1月—2022年12月阜阳市妇女儿童医院康复医学科收治的脑卒中患者91例,分为对照组(45例)与联合组(46例),方法为随机数字表法。对照组采用康复疗法治疗,联合组给予针灸联合康复疗法,两组均治疗4周。比较两组治疗4周后疗效,治疗前、治疗4周后中医证候积分、运动诱发电位、神经功能、运动功能及血清指标。结果治疗4周后,联合组总有效率更高(93.48%vs 73.33%,P<0.05)。与治疗前比较,治疗4周后两组半身不遂、眩晕、心烦易怒、言语不清、盗汗、面色苍白中医证候评分,美国国立卫生研究院卒中量表(NIHSS)评分,血清同型半胱氨酸(Hcy)、环磷酸鸟苷(cGMP)水平降低,且联合组更低(P<0.05)。与治疗前比较,治疗4周后两组潜伏期、中枢运动传导时间缩短,且联合组更短(P<0.05)。与治疗前比较,治疗4周后两组波幅、肢体运动功能运动量表(FMA)、Berg平衡量表(BBS)评分,血清环磷酸腺苷(cAMP)水平升高,且联合组更高(P<0.05)。结论针灸联合康复疗法治疗脑卒中患者肢体功能障碍的疗效确切,可降低中医证候积分,促进神经功能、运动功能恢复,分析与其改善运动诱发电位、调整血清指标有关。Objective To evaluate the effect of acupuncture combined with rehabilitation therapy on limb dysfunction in stroke patients.Methods From January 2020 to December 2022,91 stroke patients admitted to the Rehabilitation Medicine Department of Fuyang Women and Children′s Hospital were divided into the control group(45 cases)and the combined group(46 cases),using random number table method.The control group was treated with rehabilitation therapy,and the combined group was treated with acupuncture and rehabilitation therapy.Both groups were treated for 4 weeks.The efficacy after 4 weeks of treatment,the scores of traditional Chinese medicine syndrome,motor evoked potential,nerve function,motor function and serum index before and after 4 weeks of treatment of the two groups were compared.Results After 4 weeks of treatment,the total effective rate of the combined group was higher(93.48%vs 73.33%,P<0.05).Compared with those before treatment,the scores of hemiplegia,vertigo,irritability,slurred speech,night sweat,pale face,traditional Chinese medicine symptoms and national institutes of health stroke scale(NIHSS),the serum levels of homocysteine(Hcy)and cyclic guanyl phosphate(cGMP)after 4 weeks of treatment in the two groups decreased,and those in the combined group were lower(P<0.05).Compared with those before treatment,after 4 weeks of treatment,the latency and central motor conduction time of the two groups were shortened,and those of the combined group were shorter(P<0.05).Compared with those before treatment,after 4 weeks of treatment,the amplitude,fugl-meyer assessment(FMA)and Berg balance scale(BBS)of the two groups and the level of serum cyclic adenosine phosphate(cAMP)increased,and those of the combined group were higher(P<0.05).Conclusion Acupuncture and moxibustion combined with rehabilitation therapy was effective in the treatment of limb dysfunction in stroke patients,which could reduce the scores of traditional Chinese medicine symptoms,promote the recovery of nerve function and motor function,and might b
