摘　　要：目的 统计分析某医院肝移植患者的特征和费用结构变动度,针对防治肝移植疾病和减轻患者住院负担提供相关依据。方法 分析2021—2023年某医院298例肝移植患者的病案信息。结果 男性为主,达到240例(80.54%),平均年龄42.28岁,平均住院日43.26d。主要是肝细胞癌和不可逆肝病,基础病:糖尿病、高血压等;并发症:门静脉高压、门静脉血栓等。平均住院费用554 240.35元,2021—2023年平均住院日呈下降趋势,住院费用随着并发症例数呈先上涨后下降趋势;除其他类费用,西药、血液和血液制品、耗材占比排前三,年度变动最大,总变动贡献率分别为18.53%、9.45%、9.15%,变动呈下降趋势;其他类费用包含肝源和供体费用,导致占比畸高。结论 建议通过多方面的政策措施减轻患者经济负担,提升本学科在区域内的影响力。Objectiv:To statistically analyze the characteristics and the degrees of variation in expense structure of livertransplant patients in a certain hospital,and provide the relevant basis for preventimg and reatimg liver transplant diseases,andreducing patient hospitalization burden.Method:The medical record imformation of 298 liver transplant patients in a certain hospitalfrom 2021 to 2023 was analyzed.Result:Male dominated,240 male patients(80.54%),at an average age of 42.28,experienced anaverage length of hospital stay of 43.26 days.Mainly,they suffered from hepatocellular carcinoma and irreverible liver disease,ndthe basic diseases:diabetes,hiypertension,etc.;The ususal complications are portal hypertension,portlal vein thrombosis,etc.Theaverage hospitalization cost was 554,240.35 yuan,and the average hospitalization days showed a decreasing trend from 2021 to2023 and the hospitalization cost first increased and then decreased with the number of complications.In addition to other types ofexpenses,the expeses for westerm medicine,blood and blood products,consumables accounted for the top three,indicatimg thelargest annual change.Te total change contribution rates were 18.53%,9.45%,9.15%e,respectively,and the change showed adownward trend.Conclusion:Other types of expenses include liver source and donor cost,resulting in a disproportionately highproportion.It is suggested that various policy measures should be taken to reduce the economic burden of patients and enhance theinfluence of this discipline in the region.
