摘 要:目的探讨甲强龙应用史对大鼠颅骨骨缺损修复的影响和持续时间,以及对大鼠股骨骨折愈合过程的影响。方法将购自西安交通大学实验动物中心100只10周龄雄性大鼠按随机数字表法分为甲强龙组和对照组(各50只),甲强龙组给予甲强龙20 mg/kg臀肌注射4次(次/间隔2 d),对照组以生理盐水1 ml/kg代替。30只甲强龙组大鼠在末次给药结束后分别间隔2、8周建立颅骨骨缺损模型(各15只),造模后4、8、12周取材行Micro-CT测定,并行组织苏木精-伊红染色;20只甲强龙组大鼠在末次给药结束后间隔2周建立股骨骨折模型,术后0、14、28 d拍骨折部位X线;术后3、7、14、28 d取组织行苏木精-伊红染色观察。两组间差异采用独立样本t检验。结果甲强龙停药后间隔2周建立的大鼠颅骨缺损在4周时新生骨体积较对照组少29.6%(13.53±0.35比19.23±0.23,t=30.433,P<0.05);8周时较对照组少22.7%(17.58±1.35比22.75±0.31,t=8.346,P<0.05);12周时较对照组少12.5%(22.81±1.22比26.06±0.71,t=5.148,P<0.05)。停药间隔8周建立的大鼠颅骨缺损修复情况较对照组均无差异(术后4周:14.26±1.63比14.52±0.77,t=0.323,P>0.05;术后8周:19.92±1.94比22.08±3.25,t=1.276,P>0.05:术后12周:28.36±5.70比28.60±3.64,t=0.079,P>0.05)。股骨骨折模型术后14 d甲强龙组骨折断端软骨骨痂明显多于对照组[软骨面积:(1.76±0.82)mm 2比(0.31±0.54)mm 2,t=3.302,P<0.05];股骨骨折模型术后28 d X线摄片甲强龙组骨折断端仍可见骨折线,对照组已有梭形高密度骨痂连接。结论甲强龙应用史会抑制骨缺损修复,其对骨修复的抑制作用持续时间超过2周小于8周;甲强龙应用史对骨折愈合过程的影响为延缓早期炎性反应转化和编织骨转化,并抑制软骨痂形成和骨折断端吸收。Objective To investigate the effect of methylprednisolone application history on skull defect repair and the duration of its pharmacological action,as well as the healing process of femoral fractures in rats.Methods In total,10010-week-old male rats purchased from the Experimental Animal Center of Xi’an Jiaotong University were randomly assigned into a methylprednisolone group and a control group(50 rats in each group)according to the random number table method.The methylprednisolone group was intramuscularly injected methylprednisolone 20 mg/kg 4 times(times/interval 2 days),and the control group was given 1 ml/kg saline.A total of 30 rats in the methylprednisolone group were established for calvarial defects at intervals of 2 or 8 weeks after the last injection.Specimens collected at post-surgery 4,8 and 12 weeks undergo Micro-CT scan and hematoxylin-eosin(HE)staining.A closed fracture of the unilateral middle femur was created in 20 rats of the methylprednisolone group,and sequentially had open reduction and intramedullary fixation.X-ray examination was taken on the postoperative 14 and 28 days to observe the fracture healing.Fracture tissues were collected at 3,7,14 and 28 days after surgery for HE staining.The differences between the two groups were analyzed using independent sample t test.Results The new bone volume of rat calvarial defects created 2 weeks after methylprednisolone withdrawal was 29.6%less than that of the control group at 4 weeks(13.53±0.35 vs.19.23±0.23,t=30.433,P<0.05);22.7%less than that of the control group at 8 weeks(17.58±1.35 vs.22.75±0.31,t=8.346,P<0.05);and 12.5%less than that of the control group at 12 weeks(22.81±1.22 vs.26.06±0.71,t=5.148,P<0.05).The repair of calvarial defects in rats with an 8-week drug withdrawal interval was not different from that in the control group(4 weeks after surgery:14.26±1.63 vs.14.52±0.77,t=0.323,P>0.05;8 weeks after surgery:19.92±1.94 vs.22.08±3.25,t=1.276,P>0.05;12 weeks after surgery:28.36±5.70 vs.28.60±3.64,t=0.079,P>0.05).In the
