64排螺旋CT冠脉CTA在心率70~75次/min人群中的应用及社会价值

摘　　要：目的回顾性研究64排螺旋CT冠状动脉(冠脉)CT血管造影(CTA)在心率70~75次/min人群中常规应用的可行性,并研究其社会意义。方法选择260例疑似冠状动脉粥样硬化性心脏病(冠心病)患者,运用随机数字表法将其分为研究组(n=130)与对照组(n=130)。两组均开展64排螺旋CT冠状动脉CTA检查,研究组心率基本维持在70~75次/min,对照组心率基本维持<70次/min。比较两组左冠状动脉主干和右冠状动脉主干图像CT值、图像客观评价指标及检查成功率。结果研究组左冠状动脉主干和右冠状动脉主干图像CT值分别为(321.9317±72.3587)、(295.5705±25.3213)HU,与对照组的(316.1473±94.5069)、(293.3812±29.7217)HU比较,差异没有统计学意义(P>0.05)。研究组冠状动脉窦部图像CT值、冠状动脉运动伪影占比、对比度噪声比分别为(430.602±80.013)HU、11.5%、(30.124±16.207),与对照组的(448.303±74.501)HU、10.0%、(29.301±14.502)比较,差异没有统计学意义(P>0.05)。研究组检查成功率为88.5%,与对照组的90.0%比较,差异没有统计学意义(P>0.05)。结论对于疑似冠心病患者,将心率控制在70~75次/min行64排螺旋CT冠状动脉CTA检查,与心率<70次/min效果相当。Objective To retrospectively study the feasibility of 64-slice spiral CT angiography(CTA) for patients with heart rate of 70-75 beats/min and to explore its social value.Methods 260 patients with suspected coronary atherosclerotic heart disease(coronary heart disease)were selected and divided into a study group(n=130)and a control group(n=130)according to random numerical table.64-slice spiral CT coronary CTA examination was performed in both groups,and the heart rate of the study group was basically maintained at 70-75 beats/min,while that of the control group was basically maintained at<70 beats/min.The CT values of left and right main coronary artery images,objective evaluation index and examination success rate of the two groups were compared.Results The CT values of left and right main coronary artery images in the study group were(321.9317±72.3587)and(295.5705±25.3213)HU,and the differences were not statistically significant compared with(316.1473±94.5069)and(293.3812±29.7217)HU in the control group(P>0.05).The CT values of coronary sinus images,the percentage of coronary motion artifacts and the contrast-to-noise ratio in the study group were(430.602±80.013)HU,11.5%and(30.124±16.207),and the differences were not statistically significant compared with(448.303±74.501)HU,10.0%and(29.301±14.502)in the control group(P>0.05).The examination success rate of the study group was 88.5%,and the difference was not statistically significant compared with 90.0%of the control group(P>0.05).Conclusion For patients with suspected coronary artery disease,controlling the heart rate at 70-75 beats/min for 64-row spiral CT coronary CTA examination is comparable to the effect of heart rate<70 beats/min.
