高频超声联合彩色多普勒超声在肠套叠诊断及复位治疗中的应用价值分析

摘　　要：目的 探究高频超声联合彩色多普勒超声在肠套叠诊断及复位治疗中的应用价值。方法 选取84例疑似肠套叠患儿为研究对象,均接受高频超声、彩色多普勒超声检查及辅助复位治疗,以超声监测下水压灌肠结果作为诊断“金标准”,比较高频超声、彩色多普勒超声单一与联合检查对肠套叠的诊断效能;分析肠套叠复位成功率。结果 超声监测下水压灌肠结果显示:阳性69例,阴性15例;高频超声诊断结果显示:阳性67例,阴性17例;彩色多普勒超声诊断结果显示:阳性65例,阴性19例;联合检查诊断结果显示:阳性70例,阴性14例。高频超声对肠套叠的诊断准确度、灵敏度、特异度分别为92.86%、94.20%、86.67%;彩色多普勒超声对肠套叠的诊断准确度、灵敏度、特异度分别为88.10%、89.86%、80.00%;联合检查对肠套叠的诊断准确度、灵敏度、特异度分别为98.81%、100.00%、93.33%;联合检查诊断的准确度较彩色多普勒超声更高(P<0.05);联合诊断的灵敏度较高频超声、彩色多普勒超声更高(P<0.05)。69例肠套叠患儿,66例经高频超声引导下生理盐水灌肠复位成功,复位成功率为95.65%。结论 在肠套叠诊断及复位治疗中,高频超声联合彩色多普勒超声检查可提升诊断效能及治疗效果。Objective To investigate the application value of high-frequency ultrasound combined with color Doppler ultrasound in diagnosis and reduction of intussusception.Methods A total of 84 children with suspected intussusception were selected as the study subjects.All of them received high-frequency ultrasound,color Doppler ultrasound and assisted reduction treatment.The results of water pressure enema under ultrasonic monitoring were used as the "gold standard" to compare the diagnostic efficacy of high-frequency ultrasound and color Doppler ultrasound alone and in combination for intussusception was compared;the success rate of intussusception reduction was analyzed.Results The results of water pressure enema under ultrasonic monitoring showed 69 positive cases and 15 negative cases;high-frequency ultrasound showed 67 positive cases and 17 negative cases;color Doppler ultrasound showed 65 positive cases and 19 negative cases;the combined detection showed 70 positive cases and 14 negative cases.The diagnostic accuracy,sensitivity and specificity of high-frequency ultrasound for intussusception were 92.86%,94.20% and 86.67%;the diagnostic accuracy,sensitivity and specificity of color Doppler ultrasound for intussusception were 88.10%,89.86% and 80.00%;the diagnostic accuracy,sensitivity and specificity of combined detection for intussusception were 98.81%,100.00%and 93.33%.The accuracy of the combined detection was higher than color Doppler ultrasound(P<0.05).The sensitivity of the combined detection was higher than that of high-frequency ultrasound and color Doppler ultrasound alone(P<0.05).Among 69 children with intussusception,66 cases were successfully reduced by ultrasound-guided saline enema,and the success rate of reduction was 95.65%.Conclusion In the diagnosis and reduction of intussusception,high-frequency ultrasound combined with color Doppler ultrasound can improve the diagnostic efficacy and therapeutic effect.
