摘　　要：目的:探究鼻内窥镜鼻中隔黏膜下切除术与鼻内窥镜下鼻中隔成形术治疗鼻中隔偏曲(NSD)的临床效果对围术期相关指标、鼻中隔结构参数及并发症发生率的影响。方法:选取2022年1月至2024年1月永煤集团总医院收治的81例NSD患者临床资料,开展回顾性研究。根据手术方法分为观察组(n=42)和对照组(n=39),观察组采用鼻内窥镜下鼻中隔成形术,对照组采用鼻内窥镜下鼻中隔黏膜下切除术。比较两组围手术期指标、治疗效果、手术前后鼻中隔结构参数[鼻背偏离值、偏曲侧鼻腔的最小横截面积(MCA)]、通气功能[鼻腔阻力(NR)、鼻腔呼气量(EV)]、临床症状评分及术后并发症情况。结果:观察组术中出血量较对照组降低,手术时间较对照组延长,鼻腔通气恢复时间、鼻黏膜恢复时间、住院时间较对照组缩短(P<0.05);观察组总有效率95.24%(40/42)与对照组79.49%(31/39)比较,无显著差异(P>0.05);术后1个月,观察组鼻背偏离值、NR小于对照组,MCA、EV大于对照组,鼻出血、鼻塞、头痛评分低于对照组(P<0.05);观察组术后并发症发生率4.76%(2/42)与对照组12.82%(4/39)比较,无显著差异(P>0.05)。结论:鼻内窥镜下鼻中隔成形术治疗NSD手术时间较长,但能恢复鼻中隔结构,改善鼻通气功能,减轻患者症状,可确保安全性。Objective:To investigate the clinical effects of endoscopic submucous resection of the nasal septum and nasal endoscopic septoplasty on perioperative related indicators,nasal septal structure parameters,and complication rates in the treatment of nasal septal deviation(NSD).Methods:The clinical data of 81 patients with NSD admitted to Yongmei Group General Hospital from January 2022 to January 2024 were selected and retrospectively studied.They were divided into an observation group(n=42)and a control group(n=39)according to the surgical methods.The observation group underwent nasal endoscopic septoplasty,while the control group underwent endoscopic submucous resection of the nasal septum.The perioperative indicators and treatment effect were compared between the two groups,as well as nasal septum structural parameters[nasal dorsum deviation,minimum cross-sectional area(MCA)of deviated nasal cavity],[ventilation function[nasal resistance(NR),nasal expiratory volume(EV)]]and clinical symptom score before and after operation,and postoperative complications were compared between the two groups.Results:The intraoperative bleeding in the observation group was lower than that in the control group,the operation time was longer than that in the control group,and the recovery time of nasal ventilation,the recovery time of nasal mucosa,and the hospitalization time were shorter than that in the control group(P<0.05);the total effective rate in the observation group was 95.24%(40/42),and 79.49%(31/39)in the control group,with no significant difference(P>0.05);at 1 month after operation,the deviation value and NR of nasal dorsum in the observation group were less than those in the control group,MCA and EV were greater than those in the control group,and the scores of epistaxis,nasal congestion and headache in the observation group were lower than those in the control group(P<0.05);the incidence of postoperative complications in the observation group was 4.76%(2/42),which was not significantly different from 12.82%(4/39)in the
