瓜蒌薤白半夏汤合当归四逆汤加减治疗寒凝心脉型冠心病的疗效观察

摘　　要：目的:探讨瓜蒌薤白半夏汤合当归四逆汤加减治疗寒凝心脉型冠心病(CHD)的疗效,为此类患者用药提供参考。方法:选取2021年1月至2024年3月洛阳市第三人民医院收治的64例寒凝心脉型CHD患者,分为采用丹蒌片的对照组29例及采用瓜蒌薤白半夏汤合当归四逆汤的观察组35例,两组患者均联合西医基础治疗。对比两组患者治疗前及治疗3个月后心绞痛发作情况、心功能指标及血液流变学指标变化,评估治疗方案安全性。结果:治疗后两组心绞痛发作次数均减少,发作持续时间均缩短,且观察组上述两指标均低于对照组(P<0.05);治疗后两组左室舒张末期内径(LVEDD)、左室射血分数(LVEF)、心排血量(CO)均得到改善,与对照组相比,观察组治疗后LVEDD更短,LVEF、CO水平更高(P<0.05);治疗后两组血液流变学指标(血浆黏度、血小板聚集率、纤维蛋白原水平)均有降低,且观察组较对照组更低(P<0.05);治疗期间仅观察组出现1例皮疹。结论:瓜蒌薤白半夏汤合当归四逆汤加减治疗有助于提高对寒凝心脉型CHD患者的治疗效果,能减少患者心绞痛发作次数及持续时间、改善患者心功能及血液循环,安全性较高。Objective:To explore the efficacy of modified Gualou Xiebai Banxia Decoction combined with Danggui Sini Decoction in the treatment of coronary heart disease(CHD)of cold-coagulating heart pulse type,and provide a reference for medication use in such patients.Methods:Collected 64 patients with cold-coagulation heart pulse type CHD admitted to Luoyang Third People's Hospital from January 2021 to March 2024,and divided them into a control group(n=29)who received Danlou Tablets and a observation group(n=35)who received Gualou Xiebai Banxia Decoction combined with Danggui Sini Decoction.Patients in both groups were combined with basic Western medicine treatment.Compare the angina pectoris attacks,changes in cardiac function indicators and hemorrheology indicators between the two groups of patients before treatment and 3 months after treatment,evaluate the safety of the treatment regimen.Results:After treatment,the number of angina pectoris attacks was reduced and the duration of attacks was shortened in both groups,and the above two indicators in the observation group were lower than those in the control group(P<0.05).After treatment,the left ventricular end-diastolic diameter(LVEDD),left ventricular ejection fractions(LVEF)and cardiac output(CO)were improved;Compared with the control group,the observation group had shorter LVEDD and higher LVEF and CO levels after treatment(P<0.05).Hemorrheology indicators(plasma viscosity,platelet aggregation rate,and fibrinogen level)of the two groups were decreased after treatment,and the observation group was lower than the control group(P<0.05);only 1 case of rash occurred in the observation group during treatment.Conclusion:The modified treatment of Gualou Xiebai Banxia Decoction combined with Danggui Sini Decoction can help improve the therapeutic effect on CHD patients with cold-coagulation heart pulse type.It can reduce the number and duration of angina pectoris attacks,improve the patient’s cardiac function and blood circulation,and has high security.
