骨科ICU患者家属迁移应激水平与社会支持、应对方式、自我效能的关系及临床价值

摘　　要：目的:探讨骨科ICU患者家属迁移应激水平与社会支持、应对方式、自我效能的关系及临床价值。方法:选取河南省洛阳正骨医院(河南省骨科医院)2020年8月至2023年12月于骨科ICU接受治疗患者的家属120例作为研究对象,按照迁移应激水平分为低水平组(n=33)、中等水平组(n=68)和高水平组(n=19)。结果:高水平组的FRSS评分(迁移准备、家属压力、患者自护能力、家属压力及总分)、FRSS评分(观支持、客观支持、利用度及总分)、MCMQ评分(屈服、面对、回避及总分)、GSES总分均低于中等水平组和低水平组(P<0.05)。FRSS总分与SSRS总分、MCMQ总分、GSES总分呈正相关(P<0.05)。应对方式与自我效能在骨科ICU患者家属的社会支持与家属迁移应激关系间起中介效应(P<0.05)。结论:骨科ICU患者家属的社会支持越高,家属迁移应激水平越低,社会支持可直接影响家属迁移应激水平,也能通过应对方式、自我效能间接作用于家属迁移应激水平。Objective:To investigate the relationship between family migration stress level,social support,coping style and self-efficacy of patients in orthopaedic ICU and its clinical value.Methods:A total of 120 family members of patients treated in orthopaedic ICU of Luoyang Orthopedic Hospital(Henan Orthopedic Hospital)from August 2020 to December 2023 were selected as the study subjects.According to the level of migration stress,they were divided into low level group(n=33),medium level group(n=68)and high level group(n=19).Results:The FRSS score(migration preparation,family pressure,patient self-care ability,family pressure and total score),FRSS score(outlook support,objective support,utilization and total score),MCMQ score(yield,face,avoidance and total score)and total GSES score of the high level group were lower than those of the medium level group and low level group(P<0.05).The total score of FRSS was positively correlated with the total score of SSRS,MCMQ and GSES(P<0.05).The mediating effect of coping style and self-efficacy on the relationship between social support and migration stress of family members of orthopaedic ICU patients(P<0.05).Conclusion:The higher the social support of family members of patients in orthopaedic ICU,the lower the level of family members'migration stress.Social support can directly affect the level of family members'migration stress,and can also indirectly affect the level of family members'migration stress through coping styles and self-efficacy.
