小檗碱预处理对体外循环下心脏瓣膜置换术心肌保护作用的研究

摘　　要：目的:探讨小檗碱预处理对体外循环下心脏瓣膜置换术患者心肌保护作用。方法:选择体外循环下行瓣膜置换术患者19例,随机分为观察组8例,对照组11例。观察组术前7 d起每日常规治疗+小檗碱片(0.3~0.6 g/次,3次/d),术前2 h给予小檗碱片0.3~0.6 g加服一次。对照组同期给予相同剂量安慰剂。比较两组射血分数(EF)、凋亡指数(AI)、血浆心肌肌钙蛋白(cTn)T浓度及血浆肌酸激酶同工酶(CK-MB)水平。结果:两组不同时间点cTnT、CK-MB水平差异有统计学意义(P<0.05);观察组T1时EF显著高于对照组,AI显著低于对照组,差异有统计学意义(P<0.05)。结论:小檗碱预处理对行体外循环下心脏瓣膜置换术患者的心肌具有保护作用。Objective To investigate the therapeutic effects of berberine on patients undergoing valve replacement surgery under cardiopulonary bypass.Methods A total of 68 patients with extracorporeal valve replacement in our hospital from July 1,2019 to June 2024 were divided into two groups by random number table method.The control group(34 cases)received placebo.The study group(34 cases)was treated with berberine tablets.Hemodynamic indexes,vascular endothelial function,blood lipid level,imaging indexes,blood indexes and myocardial cell apoptosis index were compared between the two groups.Results After treatment,the hemodynamic indexes of the study group were better than those of the control group(P<0.05).After treatment,NO,LVEF,CI and FS in the study group were higher than those in the control group,and ET-1,TC,HDL-C,LDL-C,TG,MEE,NT-proBNP,CK-MB,cTN1 and AI were lower than those in the control group(P<0.05).Conclusions The application of berberine hydrochloride in patients with valve replacement under cardiopulmonary bypass has a good effect,can better improve the hemodynamic indexes,lipid levels,cardiac function,and reduce the myocardial apoptosis index of patients,and is worthy of clinical application.
