中药热熨经筋及筋结点干预卒中后肩手综合征患者的效果观察

摘　　要：目的观察中药热熨经筋及筋结点对卒中后肩手综合征患者疼痛、上肢运动功能和生活质量的影响。方法选取安徽省中医院针灸康复科2023年8—12月住院卒中后肩手综合征患者共68例,按照组间基线资料可比的原则分为对照组及观察组,各34例。对照组实施常规治疗及护理,观察组在对照组基础上采用经筋及筋结点中药热熨,每天1次,每次15 min,连续干预3周。于干预前、干预1周后及干预3周后评估患者疼痛评分、上肢运动功能及生活质量。结果随着干预时间的延长,两组疼痛评分逐渐降低(P时间<0.05),干预1周后、3周后,观察组疼痛评分低于对照组、观察组U-FMA评分及SS-QOL评分均低于对照组,且随着时间增长差异逐渐增大,差异有统计学意义(P交互<0.05)。中药热熨经筋及筋结点能有效改善卒中后肩手综合征患者疼痛、运动功能及生活质量,且随着时间延长效果更为明显。结论中药热熨经筋及筋结点能有效改善脑卒中后肩手综合征患者疼痛、上肢运动功能和生活质量。Objective To observe the eff ects of traditional Chinese medicine hot ironing of muscle regions and muscle joint node on pain,upper limb motor function and quality of life in patients with shoulder-hand syndrome after stroke.Methods A total of 68 patients with post-stroke shoulder-hand syndrome in the Department of Acupuncture and Rehabilitation of Anhui Traditional Chinese Medicine Hospital from August to December 2023 were selected and divided into a control group and an observation group,each with 34 cases,in accordance with the principle of comparable baseline data between the groups.Conventional treatment and nursing were implemented on the control group,and in the observation group,traditional Chinese medicine hot ironing was applied to the muscle regions and muscle joint node once a day for 15 min,continue for 3 weeks.The patients'pain scores,upper limb motor function and quality of life were assessed before,after 1 week and after 3 weeks of intervention.Results The pain scores of two groups gradually decreased with the intervention time(P<0.05).After 1 week and 3 weeks of intervention,The pain score of the observation group was lower than the control group,the U-FMA scores and SS-QOL scores of the observation group were lower than the control group,and the differences gradually increased with the increase of time(P<0.05).Hot ironing of the muscle and muscle joint nodes with traditional Chinese medicine can eff ectively release the pain,improve motor function and quality of life of patients with shoulder-hand syndrome after stroke,and the eff ect increase with time.Conclusion Hot ironing of muscle regions and muscle joint node with traditional Chinese medicine could eff ectively improve the pain,motor function of upper limbs and quality of life of patients with shoulder-hand syndrome after stroke.
