缺血性脑病采用常规CT头颈CTA检查的价值评估平扫联合

摘　　要：分析针对缺血性脑病患者实施常规CT平扫联合头颈CT血管成像(computed tomography angiography,CTA)检查的价值。研究时间为2023年1月—2024年5月,研究对象为80例疑似缺血性脑病患者,所有患者均接受常规CT检查与头颈CTA检查。以数字减影血管造影术(digital subtraction angiography,DSA)作为诊断金标准,比较单一检查、联合检查的诊断效能、颈动脉狭窄检出率、斑块检出率。结果显示,常规CT检查对疾病的诊断灵敏度、特异度、准确率分别为79.63%(43/54)、73.08%(19/26)、77.50%(62/80);头颈CTA检查对疾病的诊断灵敏度、特异度、准确率分别为81.48%(44/54)、69.23%(18/26)、77.50%(62/80);联合检查的诊断灵敏度、特异度与准确率分别为96.30%(52/54)、96.15%(25/26)、96.25%(77/80),联合检查的诊断效能、颈动脉狭窄检出率、斑块检出率均优于单一检查(P<0.05)。研究发现,于缺血性脑病患者的诊断中应用常规CT平扫联合头颈CTA检查的价值较高,值得推广。This study aims to analyze the value of performing routine CT plain scan combined with head and neck computed tomography angiography(CTA)in patients with ischemic encephalopathy.From January 2023 to May 2024,80 patients suspected of having ischemic encephalopathy were included in the study.All patients underwent routine CT examination and head and neck CTA examination.Digital subtraction angiography(DSA)was used as the gold standard for diagnosis.The diagnostic efficiency,detection rate of carotid artery stenosis,and detection rate of plaques were compared among single examination and combined examination.The results showed that the diagnostic sensitivity,specificity,and accuracy of routine CT examination for the disease were 79.63%(43/54),73.08%(19/26),and 77.50%(62/80)respectively;the diagnostic sensitivity,specificity,and accuracy of head and neck CTA examination for the disease were 81.48%(44/54),69.23%(18/26),and 77.50%(62/80)respectively;the diagnostic sensitivity,specificity,and accuracy of the combined examination were 96.30%(52/54),96.15%(25/26),and 96.25%(77/80)respectively.The diagnostic efficiency,detection rate of carotid artery stenosis,and detection rate of plaques of the combined examination were all better than those of single examination(P<0.05).The study found that the application of routine CT plain scan combined with head and neck CTA in the diagnosis of patients with ischemic encephalopathy has high value and is worthy of promotion.
