Wiltse入路与切开复位椎弓根钉内固定术+椎板切除减压术治疗胸腰椎骨折的效果研究

摘　　要：探究Wiltse入路与切开复位椎弓根钉内固定术+椎板切除减压术治疗胸腰椎骨折的疗效。选取医院2023年1月—2024年8月收治的50例胸腰椎骨折手术患者为研究对象,随机将其分为研究组和对照组,每组各25例。研究组采取Wiltse入路方式治疗,对照组采取切开复位椎弓根钉内固定术+椎板切除减压术治疗,对比两组患者的手术指标、疼痛情况、椎体功能及生活质量。结果显示,研究组手术时间较对照组短,术中出血量及术后引流量较对照组少(P<0.05)。研究组术后24 h、48 h、72 h的疼痛评分较对照组低(P<0.05)。两组患者的椎间隙高度、Cobb角比较,无统计学差异(P>0.05)。治疗后,研究组生活质量各维度评分均较对照组高(P<0.05)。研究发现,对胸腰椎骨折患者实施Wiltse入路治疗,其整体治疗效果理想,有助于改善患者的预后,建议临床推广实施。To explore the curative effect of the Wiltse approach and open reduction with pedicle screw internal fixation plus laminectomy decompression in the treatment of thoracolumbar fractures.A total of 50 patients undergoing thoracolumbar fracture surgery admitted to the hospital from January 2023 to August 2024 were selected as the research objects.They were randomly divided into a study group and a control group,with 25 patients in each group.The study group was treated with the Wiltse approach,and the control group was treated with open reduction with pedicle screw internalfixation plus laminectomy decompression.The surgical indexes,pain conditions,vertebral body functions and quality of life of the two groups of patients were compared.The results showed that the operation time of the study group was shorter than that of the control group,and the intraoperative blood loss and postoperative drainage volume were less than those of the control group(P<0.05).The pain scores of the study group at 24 h,48 h and 72 h after the operation were lower than those of the control group(P<0.05).There was no statistical difference in the intervertebral space height and Cobb angle between the two groups of patients(P>0.05).After treatment,the scores of each dimension of the quality of life in the study group were higher than those in the control group(P<0.05).The study found that the overall treatment effect of the Wiltse approach for patients with thoracolumbar fractures is ideal,which is helpful to improve the prognosis of patients,and it is recommended to be clinically promoted and implemented.
