摘  要:目的采用任务态功能性磁共振成像观察针刺过程中丘脑功能连接的性别差异。方法针刺25例男性和21例女性受试者的左侧太冲穴,采用美国马萨诸塞州总医院针感量表(Massachusetts general hospital acupuncture sensation scale,MASS)观察受试者针感的性别差异,采用模块化任务态功能性磁共振成像,以双侧丘脑为种子点,观察丘脑与全脑之间功能连接度的性别差异。结果女性受试者MASS的酸痛、疼痛、锐痛、刺痛、麻木评分均显著高于男性(P<0.05),而压力、沉重、胀感评分均显著低于男性(P<0.05)。与男性组比较,女性组左侧丘脑与双侧尾状核、右侧楔前叶、右侧距状裂、双侧舌回、左侧中央后回、左侧补充前运动区的功能连接度增强,右侧丘脑与左侧距状裂、右侧后扣带回、右侧舌回、左侧楔前叶、左侧中央旁小叶功能连接度增强。结论针刺左侧太冲穴的针感存在性别差异,其机制在于丘脑与其他脑区的功能连接发生改变。Objective To investigate the sex differences in functional connectivity of the thalamus during acupuncture based on task-state functional magnetic resonance imaging.Methods Acupuncture at left Taichong was performed for 25 male subjects and 21 female subjects,and the Massachusetts General Hospital Acupuncture Sensation Scale(MASS)was used to observe the sex differences in needling sensation.With both thalami as the seed region,modular task-state functional magnetic resonance imaging was used to observe the sex differences in the functional connectivity between the thalamus and the whole brain.Results Compared with the male subjects in terms of MASS,the female subjects had significantly higher scores of soreness,bodily pain,sharp pain,tingling,numbness,and feeling of distension(P<0.05)and significantly lower scores of pressure and heaviness(P<0.05).Compared with the male subjects,the female subjects showed enhanced functional connectivity between the left thalamus and bilateral caudate nuclei/the right precuneus/the right calcarine fissure/bilateral lingual gyri and between the right thalamus and the left calcarine fissure/right posterior cingulate gyrus/left precuneus.Conclusion There are sex differences in needling sensation during acupuncture at left Taichong,which may be caused by the changes in functional connectivity between the thalamus and other related brain regions.
