益肾泄浊汤联合隔盐灸治疗糖尿病肾病临床观察

摘　　要：目的观察益肾泄浊汤联合隔盐灸治疗糖尿病肾病的临床疗效。方法将60例脾肾亏虚型糖尿病肾病患者随机分为对照组与治疗组,每组30例,两组患者均予以西医常规治疗,对照组在常规治疗的基础上采用隔盐灸治疗,治疗组在对照组治疗基础上加用中药方剂益肾泄浊汤进行治疗,疗程均为4周。比较两组治疗前后尿白蛋白与尿肌酐比值(urinary albumin/creatinine ratio,UACR)、肾小球滤过率估算值(estimated glomerular filtration rate,eGFR)、血清肌酐(serum creatinine,SCr)、血尿素氮(blood urea nitrogen,BUN)、血β2-微球蛋白(β2-microglobulin,β2-MG)、血清胱抑素C(cystatin C,Cys-C)、空腹血糖(fasting blood glucose,FBG)、糖化血红蛋白(hemoglobin A1c,HbA1c)、三酰甘油(triglyceride,TG)、总胆固醇(total cholesterol,TC)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、餐后2 h血糖(2-hour plasma glucose,2hPG)水平,并比较两组临床疗效以及不良反应。结果治疗组临床疗效优于对照组(P<0.05)。与治疗前比较,治疗后两组患者肾功能指标中UACR、SCr、BUN、β2-MG、Cys-C水平均显著下降(P<0.05),eGFR水平显著升高(P<0.05),血糖指标中FBG、2hPG、HbA1c水平均显著下降(P<0.05),血脂指标中TG水平显著下降(P<0.05);治疗组UACR、SCr、BUN、β2-MG、Cys-C、FBG、2hPG、HbA1c以及TG水平改善程度均显著优于对照组(P<0.05)。两组均未见不良反应。结论益肾泄浊汤联合隔盐灸治疗糖尿病肾病疗效显著,且安全性高。Objective To investigate the clinical effect of Yishen Xiezhuo Decoction combined with moxibustion on salt in the treatment of diabetic nephropathy.Methods A total of 60 patients with diabetic nephropathy with spleen-kidney deficiency were randomly divided into control group and treatment group,with 30 patients in each group.The patients in the control group were given conventional Western medicine treatment combined with moxibustion on salt,while those in the treatment group were given the traditional Chinese medicine prescription Yishen Xiezhuo Decoction in addition to the treatment in the control group;the course of treatment was 4 weeks for both groups.The two groups were compared in terms of urinary albumin-to-creatinine ratio(UACR),estimated glomerular filtration rate(eGFR),serum creatinine(SCr),blood urea nitrogen(BUN),β2-microglobulin(β2-MG),serum cystatin C(Cys-C),fasting blood glucose(FBG),hemoglobin A1c(HbA1c),triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),and 2-hour postprandial glucose(2hPG),and clinical outcome and adverse events were also compared between the two groups.Results The treatment group had a significantly better clinical outcome than the control group(P<0.05).As for renal function parameters after treatment,both groups had significant reductions in the levels of UACR,SCr,BUN,β-2MG,and Cys-C(P<0.05)and a significant increase in the level of eGFR(P<0.05);as for blood glucose parameters,both groups had significant reductions in the levels of FBG,2hPG,and HbA1c(P<0.05);as for blood lipid parameters,both groups had a significant reduction in TG(P<0.05),and compared with the control group,the treatment group had significantly better improvements in UACR,SCr,BUN,β2-MG,Cys-C,FBG,2hPG,HbA1c,and TG(P<0.05).No adverse events were observed in either group.Conclusion Yishen Xiezhuo Decoction combined with moxibustion on salt has a marked clinical effect in the treatment of diabetic nephropathy,with a favorable safety
