摘 要:目的观察骨痹通消颗粒对激素性股骨头坏死(steroid-induced osteonecrosis of the femoral head,SONFH)大鼠股骨头中转化生长因子-β1(transforming growth factor-beta 1,TGF-β1)/Smads信号通路的影响。方法将30只雄性SD大鼠随机分成对照组(n=6)、模型组(n=12)、治疗组(n=12),采用脂多糖和注射用甲泼尼龙琥珀酸钠联合注射的方式复制SONFH大鼠模型。采用苏木精—伊红染色法观察大鼠股骨头的组织形态并定量分析空骨陷窝率;酶联免疫吸附试验检测血清骨碱性磷酸酶(bone alkaline phosphatase,BALP)、Ⅰ型前胶原氨基末端肽(procollagen typeⅠN-terminal propeptide,PINP)、抗酒石酸酸性磷酸酶(tartrate-resistant acid phosphatase,TRACP)、β型Ⅰ型胶原C-末端肽(beta-isomerized C-terminal telopeptide of typeⅠcollagen,β-CTX)水平;Western blot法检测股骨头中TGF-β1、Smad2、Smad3、p-Smad2、p-Smad3、Runx2蛋白表达水平;RT-qPCR法检测股骨头中TGF-β1、Smad2、Smad3、Runx2 mRNA表达水平。结果骨痹通消颗粒能显著升高SONFH大鼠股骨头中TGF-β1、Smad2、Smad3、p-Smad2、p-Smad3、Runx2蛋白及TGF-β1、Smad2、Smad3、Runx2 mRNA表达水平及血清BALP、PINP水平(P<0.05),显著降低血清β-CTX、TRACP水平和股骨头空骨陷窝率(P<0.05)。结论骨痹通消颗粒治疗SONFH的机制与激活TGF-β1/Smads信号通路、促进骨形成并抑制骨吸收有关。Objective To investigate the effect of Gubi Tongxiao Granule on the transforming growth factor-β1(TGF-β1)/Smads signaling pathway in rats with steroid-induced osteonecrosis of the femoral head(SONFH).Methods A total of 30 male Sprague-Dawley rats were randomly divided into control group with 6 rats,model group with 12 rats,and treatment group with 12 rats.A rat model of SONFH was established by combined injection of lipopolysaccharide and methylprednisolone.Hematoxylin and eosin staining was used to observe the histomorphology of the femoral head;ELISA was used to measure the serum levels of bone alkaline phosphatase(BALP),procollagen typeⅠN-terminal propeptide(PINP),tartrate-resistant acid phosphatase(TRACP),andβisomer of C-terminal telopeptide of typeⅠcollagen(β-CTX);Western blot was used to measure the protein expression levels of TGF-β1,Smad2,Smad3,p-Smad2,p-Smad3,and Runx2 in the femoral head,and RT-qPCR was used to measure the mRNA expression levels of TGF-β1,Smad2,Smad3,and Runx2 in the femoral head.Results Gubi Tongxiao Granule significantly increased the protein expression levels of TGF-β1,Smad2,Smad3,p-Smad2,p-Smad3,and Runx2 and the mRNA expression levels of TGF-β1,Smad2,Smad3,and Runx2 in the femoral head,as well as the serum levels of BALP and PINP(P<0.05),and it significantly reduced the serum levels ofβ-CTX and TRACP and the percentage of empty lacunae in the femoral head(P<0.05).Conclusion Gubi Tongxiao Granule exerts a therapeutic effect on SONFH by activating the TGF-β1/Smads signaling pathway,promoting bone formation,and inhibiting bone resorption.
