摘　　要：目前认为慢性肾脏病(chronic kidney disease,CKD)进展的根本基础为功能性肾单位的逐渐缺损及丧失,其过程与细胞凋亡密切相关。细胞凋亡是一种受调控的、能量依赖的,以半胱氨酸蛋白酶(Caspase)激活为主要特征的程序性细胞死亡形式之一,而肾小管、肾小球以及内皮在内的肾单位丧失过程都伴有细胞凋亡的参与。现已发现的核心细胞凋亡信号通路机制主要有三种类型,即Caspase依赖性细胞凋亡途径、非Caspase依赖性细胞凋亡途径以及穿孔蛋白-颗粒酶介导的细胞凋亡途径。不同细胞凋亡途径不仅是CKD发病机制的基础,而且对防治或延缓CKD进展方面具有重要的治疗意义。因此,本文就细胞凋亡在CKD方面进行综述,为CKD发病机制提供支持依据。At present,it is believed that the fundamental basis for the progression of chronic kidney disease(CKD)is the gradual deficiency and loss of functional nephrons,which is closely related to cell apoptosis.Apoptosis is one of the regulated and energy dependent forms of programmed cell death characterized by activation of cysteine protease(caspase),and the loss of renal units including tubules,glomeruli,and endothelium is accompanied by apoptosis.There are three main types of core cell apoptosis signaling pathways that have been discovered,namely caspase dependent cell apoptosis pathway,non caspase dependent cell apoptosis pathway,and perforin granzyme mediated cell apoptosis pathway.Different pathways of cell apoptosis are not only the basis of the pathogenesis of CKD,but also have important therapeutic significance in preventing or delaying the progression of CKD.Therefore,this article provides a review of the role of cell apoptosis in CKD,providing supporting evidence for the pathogenesis of CKD.
