摘 要:目的探讨ABO、RhD配合型输血与同型输血的有效性和安全性。方法选择2022年2月-2024年3月我院收治的需要输注红细胞的患者132例,以随机函数法分为对照组和观察组,每组各66例,对照组采取同型输血,观察组采取ABO、RhD配合型输血。对比两组患者输血有效性指标、间接胆红素水平,以及不规则抗体筛查和直接抗人球蛋白实验结果,并观察输血不良反应。结果输血24h后,两组患者Hb、Hct、RBC水平均高于输血前,差异有统计学意义(P<0.05),IBiL水平低于输血前,但对比差异无统计学意义(P>0.05)。输血24h后,两组患者Hb、Hct、RBC、IBiL水平对比,差异均无统计学意义(P>0.05);两组患者Rh血型系统抗体阳性率、其他血型系统抗体阳性率、DAT阳性率对比,差异均无统计学意义(P>0.05);两组输血不良反应发生率对比,差异无统计学意义(P>0.05)。结论配合型输血和同型输血均能有效改善患者血常规指标,且对不规则抗体产生的影响较小。但配合型输血不良反应更少,在血液资源有限、交叉配血不合等特殊情况下,配合型输血是一种安全有效的替代方案。Objective To investigate the efficacy and safety of ABO-RhD-matched blood transfusion and homotype blood transfusion.Methods A total of 132 patients who needed red blood cell transfusion in our hospital from February 2022 to March 2024 were divided into a control group and an observation group according to the random function method,with 66 cases in each group.The control group was treated with homotype blood transfusion,and the observation group was treated with ABO and RhD matched blood transfusion.The effectiveness indicators of blood transfusion,indirect bilirubin level,irregular antibody screening and direct anti-human globulin test results were compared between the two groups,and the adverse reactions of blood transfusion were observed.Results After 24 hours of blood transfusion,the Hb,Hct,and RBC levels of both groups of patients were higher than before transfusion(P<0.05),and the IBiL level was lower than before transfusion(P>0.05).Comparison of Hb,Hct,RBC,and IBiL levels between two groups of patients 24 hours after blood transfusion(P>0.05);comparison of Rh blood group system antibody positivity rate,other blood group system antibody positivity rate,and DAT positivity rate between two groups of patients(P>0.05);comparison of the incidence of adverse reactions to blood transfusion between two groups(P>0.05).Conclusion Both cooperative and homologous blood transfusions improve blood routine and have little impact on irregular antibodies.Cooperative blood transfusion has fewer adverse reactions and is a safe and effective alternative in special circumstances.
