多层螺旋CT诊断先天性心脏病患儿大血管畸形的价值体会

摘　　要：目的对多层螺旋CT诊断先天性心脏病患儿大血管畸形的价值进行探讨。方法选定的研究时间段为2022年12月-2023年12月。纳入我院收治的先天性心脏病患儿共105例为研究对象。在纳入的患儿中,经由手术证实,有31例为大血管畸形,其余74例为心内结构异常。回顾分析105例患儿的多层螺旋CT影像学资料,以手术结果为金标准,分析此技术的应用效能。结果①在多层螺旋CT对心内先天性心脏病的诊断中,确诊率均达到了85%以上,涵盖了主动脉骑跨、单心室、单心房、三尖瓣闭锁等各类型,与实际情况完全吻合。②应用多层螺旋CT后,在大血管部分畸形先天性心脏病的诊断中,共检测出29例,确诊率高达93.55%。多层螺旋CT的诊断准确度为93.33%(98/105),灵敏度为90.32%(28/31),特异度为94.59%(70/74)。结论在先天性心脏病患儿大血管畸形的临床诊断中,应用多层螺旋CT技术,具有较高的诊断效能,并且属于无创操作,易于被患儿接受。Objective To explore the value of multi-slice spiral CT in diagnosing large vascular malformations in children with congenital heart disease.Methods The study dates were selected from December 2022 to December 2023.A total of 105 children with congenital heart disease in our hospital were included in the study.In this part of the children,31 cases were confirmed by surgery as large vascular malformations.The remaining 74 cases were intracardiac structural abnormalities.Multi-slice spiral CT imaging data of 105 children were retrospectively analyzed.The application efficiency of this technique was analyzed with the surgical result as the gold standard.Results①In the diagnosis of intracardiac congenital heart disease by multi-slice spiral CT,the diagnosis rate reached more than 85%,covering all types of aortic straddle,single ventricle,single atrium,tricuspid valve atresia,etc.,which was completely consistent with the actual situation.②After the application of multi-slice spiral CT,29 cases of congenital heart disease with partial malformation of large vessels were detected,the diagnosis rate was as high as 93.55%.The diagnostic accuracy of MSCT was 93.33%(98/105),the sensitivity was 90.32%(28/31),and the specificity was 94.59%(70/74).Conclusion In the clinical diagnosis of large vascular malformation in children with congenital heart disease,the application of multi-slice spiral CT has high diagnostic efficiency,and is a non-invasive operation,easy to be accepted by pediatric patients.
