摘 要:目的探讨超声心动图联合心电图检查在心尖肥厚型心肌病临床诊断中的应用价值。方法选取福州市第一总医院2022年3月—2024年3月收治的30例确诊为心尖肥厚型心肌病患者作为观察组,另选取30名同期健康体检人员作为对照组,所有人员均接受超声心动图及心电图联合检查,分析2组研究对象的检查结果,并探究2种方式联合检测的价值。结果观察组T波倒置幅度、胸前导联R波振幅分别为(10.98±1.87)mV、(2.42±0.53)mV,大于对照组的(0.04±0.01)mV、(1.29±0.38)mV,ST段下移发生率为90.00%,高于对照组的0,差异均有统计学意义(P<0.05)。观察组的左心房内径(left atrial inner diameter,LAD)、心尖厚度及左心室后壁厚度(left ventricular posterior wall,LVPW)分别为(49.36±3.16)mm、(15.07±2.23)mm、(11.63±2.77)mm,大于对照组的(32.39±3.18)mm、(10.29±2.38)mm、(7.19±1.69)mm,A峰、E峰分别为(78.39±5.24)cm/s、(80.74±4.68)cm/s,高于对照组的(52.37±5.21)cm/s、(51.23±4.17)cm/s,差异均有统计学意义(P<0.05)。联合检查的检出率为93.33%,高于心电图与超声心动图的检出率56.67%、66.67%,差异均有统计学意义(P<0.05)。结论在诊断心尖肥厚型心肌病时,联合应用超声心动图与心电图2种检查方法,可从不同角度观察心脏的结构与功能,明确病变情况,具有良好的诊断价值。Objective To explore the application value of echocardiography combined with electrocardiogram in the clinical diagnosis of apical hyper-trophic cardiomyopathy.Methods From March 2022 to March 2024,30 patients diagnosed with apical hyper-trophic cardiomyopathy admitted to Fuzhou First General Hospital as the observation group,and another 30 healthy individuals who underwent physical examinations during the same period as the control group.All individuals underwent a combined examination of echocardiography and electrocardiogram.The examination results of the two groups were analyzed, and the value of the combined detection method was explored. Results The amplitude of T wave inversion and chest lead R wave in the observation group were (10.98±1.87) mV and (2.42±0.53) mV, respectively, which were higher than those in the control group (0.04±0.01) mV and (1.29±0.38) mV. The incidence of ST segment depression was 90.00%, which was higher than that in the control group (0). The differences were statistically significant (P < 0.05). The left atrial inner diameter (LAD), apex thickness, and left ventricular posterior wall (LVPW) of the observation group were (49.36±3.16) mm, (15.07±2.23) mm, and (11.63±2.77) mm, respectively, which were larger than those of the control group (32.39±3.18) mm, (10.29±2.38) mm, and (7.19±1.69) mm. The A peak and E peak were (78.39±5.24) cm/s and (80.74±4.68) cm/s, respectively, which were higher than those of the control group (52.37±5.21) cm/s and (51.23±4.17) cm/s, respectively. The differences were statistically significant (P < 0.05). The detection rate of joint examination was 93.33%, which was higher than the detection rates of electrocardiogram and echocardiography (56.67% and 66.67%, respectively), and the differences were statistically significant (P < 0.05).
