摘  要:目的本研究旨在通过认知性访谈评估和优化中文版组织变革准备度量表(the organizational readiness for change assessment,ORCA)的理解度和条目表达。方法研究采用目的抽样法,选取2023年9—10月在上海市某三级甲等儿科医院外科病房护士作为研究对象。经过3轮访谈,收集她们对量表条目的理解和反馈。通过专家小组讨论,结合访谈结果,对访谈资料进行深入分析,并据此对量表条目进行修订。结果本研究共纳入28名护士参与3轮认知性访谈。第1轮完成15例访谈资料分析,结果显示,有28个条目的句意模糊,表达不清,需要进行修订;第2轮完成8例,结果显示,11个条目表达不清且句意难以理解,需要进行修订;第3轮完成5例认知性访谈并达到资料饱和。最终版中文版ORCA量表包括3个维度,19个子量表,共65个条目。量表的整体Cronbach′sα系数为0.939,重测信度为0.916,量表条目的内容效度指数为0.778~1.000,量表平均内容效度指数为0.916。结论认知性访谈在确保量表与原量表概念和语义等价性中的重要作用,证实了认知性访谈是提高量表科学性和准确性的有效工具。Objective To evaluate and refine the comprehensibility and item expression of the Chinese version of the organizational readiness for change assessment(ORCA)scale by cognitive interview.Methods Purposeful sampling was employed to select nurses working in the surgical wards of a tertiary pediatric hospital in Shanghai between September and October 2023.Through 3 rounds of interviews,and their understanding and feedback on the scale items were collected.The interview data were thoroughly analyzed through expert panel discussions,combined with interview results,and the scale items were revised based on the findings.Results A total of 28 nurses were enrolled to participate in 3 rounds of cognitive interviews in this study.The first round involved completing 15 interviews,and the analysis of the interview data revealed that 28 items were ambiguous and poorly expressed,warranting revision.The second round included conducting 8 interviews,and the results indicated that 11 items were unclear and difficult to understand,necessitating further revision.The third round was concluded with 5 interviews and achieved data saturation.The final version of the Chinese ORCA scale encompassed 3 dimensions,19 subscales,and a total of 65 items.The overall Cronbach'sαcoefficient for the scale was 0.939,with test-retest reliability of 0.916.The content validity indices(CVIs)of the items ranged from 0.778 to 1.000,with an average CVI of 0.916.Conclusion Cognitive interviews play a vital role in ensuring the conceptual and semantic equivalence between the scale and the source scale,is confirmed that it is an effective tool to enhance scientific rigor and accuracy of the scale.
