摘  要:目的探讨分析宏基因二代测序技术(mNGS)在下呼吸道感染病原学检测中的临床价值。方法回顾性分析2021年7月至2023年9月荆州市中心医院下呼吸道感染患者资料,225例患者均进行肺泡灌洗液mNGS检测和传统培养,比较分析两种检测方法的阳性率、病原学分布和检测结果的一致性。结果225例患者中,传统培养阳性83例,其中混合感染16例,共检出103株病原菌;宏基因二代测序检测阳性199例,其中混合感染129例,共检出340株病原菌。结论在下呼吸道感染中,肺泡灌洗液mNGS对病原菌的阳性检出率明显高于微生物传统培养(P<0.05),且检测周期短,可作为传统病原学检测的补充方法,以帮助医生快速诊断和治疗疾病。Objective to explore and analyse the clinical value of macro gene second-generation sequencing(mNGS)technology in the detection of the aetiology of lower respiratory tract lung infections.Methods the data of patients with lower respiratory tract infections in Jingzhou Central Hospital were selected for retrospective analysis during the period from July 2021 to September 2023.225 patients were subjected to alveolar lavage mNGS detection and traditional culture,comparing the positivity rate,pathogenicity distribution and the consistency of the results between the two detection methods.Result Among 225 patients,83 cases were positive for traditional culture,including 16 cases of mixed infections,with a total of 103 strains of pathogens detected;199 cases were positive for mNGS test,including 129 cases of mixed infections,with a total of 340 strains of pathogens detected.Conclusion In lower respiratory tract infections,alveolar lavage mNGS has a higher positive detection rate of pathogenic bacteria than the traditional culture method of microorganisms,and the detection period is short,so it can be used as a complementary method to the traditional pathogenicity testing to help doctors quickly diagnose and treat diseases.
