1064 nm超皮秒激光联合强脉冲光治疗黄褐斑的临床研究

摘　　要：目的探讨1064 nm超皮秒激光联合强脉冲光治疗黄褐斑的临床效果。方法选取2022年1月至2023年12月于郑州壹加壹医疗美容医院就诊的黄褐斑女性患者114例,按治疗方法不同分成对照组和观察组,每组57例。对照组采用强脉冲光治疗,观察组采用超皮秒联合IPL治疗。比较两组治疗后临床疗效、不良反应率、满意度,比较两组治疗前、后的皮损评分、皮肤屏障功能[经表皮水分流失值(TEWL)、角质层含水量]及黑素含量、黄褐斑生活质量指数量表(MelasQoL)。结果观察组治疗后的总有效率为85.96%,高于对照组的64.91%,差异有统计学意义(P<0.05);治疗后,两组皮损面积及皮损颜色评分、MelasQoL评分、黑素含量均低于治疗前,且观察组分别为(0.87±0.25)分、(0.91±0.26)分、(19.52±2.49)分、(21.53±1.25)U.C,低于对照组的(1.46±0.34)分、(1.53±0.34)分、(27.84±3.17)分、(39.14±1.57)U.C,差异有统计学意义(P<0.05);两组患者治疗前、后TEWL值、角质层含水量比较,差异无统计学意义(P>0.05);观察组治疗后的满意度为(92.15±3.62)分,高于对照组的(85.94±4.53)分,差异有统计学意义(P<0.05);两组不良反应发生率分别为8.77%、7.02%,组间比较差异无统计学意义(P>0.05)。结论超皮秒联合强脉冲光可有效降低黄褐斑女性黑素含量,淡化色斑,紧致皮肤,提高皮肤光泽感,改善患者生活质量,且不会对患者皮肤屏障功能产生不利影响。Objective Exploring the clinical effificacy of 1064 nm ultra picosecond laser combined with intense pulsed light in the treatment of melasma.Methods A total of 114 female patients with melasma admitted the hospital were enrolled between January 2022 and December 2023.Divided into a control group and an observation group according to the treatment method.The control group was treated with intense pulsed light,while the observation group was treated with ultra picosecond combined with IPL.Compare the clinical effificacy,adverse reaction rate,and satisfaction of the two groups after treatment,compare the skin lesion scores,skin barrier function(TEWL,stratum corneum water content),melanin content,and melasma quality of life index(MelasQoL)before and after treatment between the two groups.Results The total effective rate of the observation group after treatment was 85.96%,which was higher than the control group's 64.91%,and the difference was statistically signifificant(P<0.05).After treatment,the area and color score of skin lesions,MelasQoL score,and melanin content in both groups were significantly reduced compared to baseline measurements.Notably,the observation group exhibited lower values than the control group[(0.87±0.25)points,(0.91±0.26)points,(19.52±2.49)points,(21.53±1.25)U.C vs(1.46±0.34)points,(1.53±0.34)points,(27.84±3.17)points,(39.14±1.57)U.C],the difference is statistically signifificant(P<0.05).There was no signifificant difference in TEWL values and stratum corneum water content between the two groups of patients before and after treatment(P>0.05).The satisfaction score of the observation group after treatment was(92.15±3.62)points,which was higher than that of the control group(85.94±4.53)points,the difference is statistically signifificant(P<0.05).The incidence of adverse reactions in the two groups was 8.77%and 7.02%,respectively,and there was no statistically signifificant difference between the groups(P>0.05).Conclusions Ultra picosecond combined with IPL treatment can effectively red
