曲安奈德联合二氧化碳点阵激光治疗增生性瘢痕的临床观察

摘　　要：目的探讨增生性瘢痕患者应用曲安奈德结合二氧化碳(CO_(2))点阵激光治疗的疗效以及对瘢痕评价量表(VSS)评分的影响。方法选取2022年1月至2023年12月在信阳市中心医院接受治疗的86例增生性瘢痕患者作为研究对象,按治疗方法不同分为对照组和观察组,每组43例,对照组患者给予CO_(2)点阵激光治疗;观察组患者43予以曲安奈德结合CO_(2)点阵激光治疗。观察两组患者的临床疗效,并对比治疗前后两组患者的炎性因子、再生相关生长因子、瘢痕血流灌注量、瘢痕厚度、温哥华瘢痕量表(VSS)评分以及不良反应发生率的具体变化。结果观察组患者的总有效率高于对照组,差异有统计学意义(P<0.05);炎性因子明显低于对照组,差异有统计学意义(P<0.05);再生相关生长因子水平明显低于对照组,差异有统计学意义(P<0.05);瘢痕血流灌注量与瘢痕厚度以及VSS评分显著低于对照组,差异有统计学意义(P<0.05)。结论应用曲安奈德结合CO_(2)点阵激光治疗增生性瘢痕患者,疗效确切,可减轻局部炎症反应,抑制瘢痕内血管新生,降低瘢痕血流灌注量,削减瘢痕厚度,促进瘢痕消退,降低不良反应发生率。Objective To investigate the efficacy of triamcinolone acetonide combined with carbon dioxide(CO_(2))dot matrix laser in the treatment of hypertrophic scar and its effect on VSS score.Methods A total of 86 patients with hypertrophic scar treated in Xinyang Central Hospital from January 2022 to December 2023 were selected as research objects and divided into control group and observation group according to different treatment methods,with 43 cases in each group.Patients in the control group were given CO_(2)dot matrix laser treatment.Observation group 43 patients were treated with triamcinolone and CO_(2)dot matrix laser.The clinical effificacy of the two groups of patients was observed,and the specifific changes of inflflammatory factors,regeneration related growth factors,scar blood perfusion volume,scar thickness,Vancouver Scar scale(VSS)score and incidence of adverse reactions were compared before and after treatment.Results The total effective rate of observation group was higher than that of control group,the difference was statistically signifificant(P<0.05).The inflflammatory factors were signifificantly lower than those in the control group,the difference was statistically signifificant(P<0.05).The level of regeneration related growth factor was signifificantly lower than that of control group,the difference was statistically signifificant(P<0.05).Scar blood perfusion volume,scar thickness and VSS score were signifificantly lower than those of control group,with statistical signifificance(P<0.05).Conclusions Triamcinolone acetonide combined with CO_(2)dot-matrix laser is effective in the treatment of hypertrophic scars.It can reduce local inflflammatory reaction,inhibit angiogenesis in scars,reduce scar blood flflow perfusion,and reduce scar thickness,promote scar regression,reduce the incidence of adverse reactions.
