摘　　要：目的探讨鱼骨式分层减张缝合联合浅层X线放射治疗在高张力部位瘢痕疙瘩手术中的应用效果。方法回顾性分析2021年5月至2023年5月济南现代皮肤病医院收治的80例胸、肩背部瘢痕疙瘩患者临床资料。根据治疗方式不同将患者分为观察组和对照组,观察组40例采用鱼骨式分层减张缝合联合浅层X线放射治疗,对照组40例采用鱼骨式分层减张缝合联合激素注射治疗方式。观察统计两组患者治疗16个月后的总体疗效情况,温哥华瘢痕量表评分情况,复发率,不良反应发生率,采用问卷调查形式统计两组外观满意度并进行对比。结果治疗后,观察组治疗总有效率和外观满意度高于对照组,症状改善情况优于对照组,观察组温哥华瘢痕量表总分和复发率低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较差异无统计学意义(P>0.05)。结论鱼骨式分层减张缝合联合浅层X线放射治疗在高张力部位瘢痕疙瘩手术治疗中有良好应用前景,能够改善患者症状,降低复发率,提升外观满意度。Objective To explore the application effect of fish bone layered tension reducing suture combined with shallow X-ray radiation therapy in high tension scar tissue surgery.Methods A retrospective analysis was carried out on the clinical data of 80 patients with keloids on the chest,shoulders and back who were admitted to Jinan Modern Skin Disease Hospital from May 2021 to May 2023.According to different treatment methods,the patients were divided into an observation group and a control group.In the observation group,40 patients received fishbone-shaped layered tension-reducing suture combined with superficial X-ray radiotherapy,while in the control group,40 patients received fishbone-shaped layered tension-reducing suture combined with hormone injection treatment.The overall curative effect,scores of the Vancouver Scar Scale,recurrence rate,and incidence of adverse reactions of the patients in the two groups 16 months after treatment were observed and statistically analyzed.The appearance satisfaction of the two groups was statistically analyzed and compared through a questionnaire survey.Results After the treatment,the total effective rate of the observation group was higher than that of the control group,the symptom improvement was better than that of the control group,the total score of the Vancouver Scar Scale was lower than that of the control group,the recurrence rate was lower than that of the control group,and the appearance satisfaction was higher than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Fish bone layered tension reducing suture combined with shallow X-ray radiation therapy has good application prospects in the surgical treatment of high tension scar tissue,which can improve patient symptoms,reduce recurrence rate,and enhance appearance satisfaction.
