颏下岛状皮瓣与前臂游离皮瓣在面部外伤致皮肤软组织缺损修复中的应用效果比较

摘　　要：目的比较颏下岛状皮瓣、前臂游离皮瓣修复面部外伤致皮肤软组织缺损的临床效果。方法选取2022年6月至2024年7月驻马店市中心医院收治的面部外伤致皮肤软组织缺损患者80例,根据修复方案不同分组,A组患者采取颏下岛状皮瓣修复,B组患者采取前臂游离皮瓣修复。比较两组患者手术时间、住院时间、术中输血量、术后疼痛评分、皮瓣存活等情况。结果与B组比较,A组手术时间与住院时间更短,术中出血量与术后VAS评分更低,差异有统计学意义(P<0.05);A组皮瓣成功40例,成功率100.00%;B组皮瓣成活38例,成活率95.00%,组间比较差异无统计学意义(P>0.05);与B组比较,A组皮瓣功能、色泽、美观度评分均更高,差异有统计学意义(P<0.05);与B组比较,A组术后并发症发生率更低,差异有统计学意义(P<0.05)。结论颏下岛状皮瓣、前臂游离皮瓣用于面部外伤致皮肤软组织缺损中均有较好的修复效果,成活率均较高,但是颏下岛状皮瓣创伤更小,术后恢复更快,外观、色泽与功能恢复更快,且并发症发生率更低,可作为首选修复方案。Objective Compare the clinical efficacy of submental island flap and forearm free flap in repairing skin and soft tissue defects caused by facial trauma.Methods Eighty patients with facial trauma induced skin and soft tissue defects admitted to Zhumadian Central Hospital from June 2022 to July 2024 were selected and divided into different groups according to the repair plan.Group A patients were treated with submental island flaps,while Group B patients were treated with forearm free flaps.Compare the surgical time,hospital stay,intraoperative blood transfusion volume,postoperative pain score,and flap survival between two groups of patients.Results Compared with Group B,Group A had shorter surgical time and hospitalization time,lower intraoperative blood loss and postoperative VAS score,and the difference was statistically significant(P<0.05);40 cases of Group A skin flaps were successful,with a success rate of 100.00%;38 cases of B group skin flaps survived,with a survival rate of 95.00%,and there was no statistically significant difference between the groups(P>0.05);Compared with Group B,Group A had higher scores in flap function,color,and aesthetics,with statistically significant differences(P<0.05);Compared with Group B,Group A had a lower incidence of postoperative complications,and the difference was statistically significant(P<0.05).Conclusions Both submental island flaps and forearm free flaps have good repair effects on skin and soft tissue defects caused by facial trauma,with high survival rates.However,submental island flaps have smaller trauma,faster postoperative recovery,faster appearance,color,and function recovery,and lower incidence of complications,making them the preferred repair options.
