富血小板纤维蛋白联合负压封闭引流在慢性创面中的应用

摘　　要：目的观察富血小板纤维蛋白(PRF)联合负压封闭引流(VSD)治疗慢性创面的临床效果。方法选取2023年3月至2024年3月达州市中心医院收治的64例慢性难愈创面患者,按治疗方法不同分为对照组(32例)和观察组(32例),观察组患者采用PRF联合VSD治疗,对照组患者采用VSD治疗,观察记录2组患者干预治疗后创面达手术修复标准时间、创面完全愈合时间、住院总费用。结果观察组患者创面达手术修复标准时间为(9.69±0.59)d,少于对照组(13.44±2.05)d,差异有统计学意义(t=9.955,P<0.001);观察组患者创面完全愈合时间观察组为(18.31±2.57)d少于对照组(24.31±4.89)d,差异有统计学意义(t=6.145,P<0.001);住院总费用观察组为(16949±5398)元少于对照组(19850±3454)元,差异有统计学意义(t=2.561,P=0.013)。结论富血小板纤维蛋白联合负压封闭引流治疗慢性创面,能加速创面肉芽组织生长、促进创面愈合、降低患者住院费用,可在基层医院推广应用。Objective To observe the clinical effect of platelet-rich fibrin(PRF)combined with vacuum sealing drainage(VSD)in the treatment of chronic wounds.Methods A total of 64 patients with chronic difficult to heal wounds admitted to the Dazhou Central Hospital from March 2023 to March 2024,who met the inclusion and exclusion criteria,were randomly divided into a control group(32 cases)and an observation group(32 cases)using a random number table method.Patients in the observation group received PRF combined with VSD treatment,while patients in the control group received VSD treatment.The time for the wound to reach the surgical repair standard,the time for complete wound healing,and the total hospitalization cost were observed and recorded for both groups of patients after intervention treatment.Results The observation showed that the time for the patient's wound to reach the surgical repair standard was(9.69±0.59)d,which was less than the control group's(13.44±2.05)d,and the difference was statistically significant(t=9.955,P<0.001);the time of complete healing of trauma in the patients of the observation group was(18.31±2.57)d in the observation group less than that in the control group(24.31±4.89)d,and the difference was statistically significant(t=6.145,P<0.001);total hospitalization cost was(16949±5398)yuan in the observation group less than(19850±3454)yuan in the control group,and the difference was statistically significant(t=2.561,P=0.013).Conclusions Platelet-rich fibrin combined with negative pressure occlusion drainage in the treatment of chronic wounds can accelerate the growth of wound granulation tissue,promote wound healing,and reduce patients'hospitalization costs,and can be popularized and applied in primary hospitals.
