摘　　要：目的观察指背分叶多边形皮瓣在小儿先天性并指畸形分指重建中的应用效果。方法选取2020年1月至2024年6月南阳市中心医院收治的指背分叶多边形皮瓣进行分指重建的先天性并指畸形患儿67例,通过术后并发症发生率、美学效果、分指功能、生活质量改变情况等指标评价其临床效果。结果所有患儿术后发生关节僵硬2例,挛缩畸形1例,并发症发生率4.48%;温哥华瘢痕评定量表平均得分为(7.83±1.24)分;指蹼爬移Withey评分量表平均得分为(1.76±0.35)分,与对侧指差异无统计学意义(P>0.05)。随访6个月,患儿生活质量简表(QOL-BREF)各维度得分及总分明显较治疗前提升,治疗前后比较差异有统计学意义(P<0.05)。结论指背分叶多边形皮瓣用于先天性并指畸形分指重建可有效改善畸形患指功能,提高整形美学效果,并发症少,安全可靠;配合术后康复干预可有效提升患儿生活质量。Objective Observation of the application effect of the dorsal divided polygonal skin flap in the reconstruction of fingers in children with congenital syndactyly.Methods A retrospective analysis was conducted on the clinical data of children with congenital syndactyly who underwent finger reconstruction using a finger dorsal lobulated polygonal flap in our hospital from January 2020 to June 2024.The clinical outcomes were evaluated based on indicators such as postoperative complication rate,aesthetic effect,finger function,and changes in quality of life.Results A total of 67 subjects were included in the study,including 39 males and 28 females,with an average age of(4.75±1.04)years.Two cases of joint stiffness and one case of contracture deformity occurred after surgery,with a complication rate of 4.48%;The average score of the Vancouver Scar Assessment Scale is(7.83±1.24)points;The average score of the Finger Web Crawling Withey Scale was(1.76±0.35)points,and there was no statistically significant difference compared to the contralateral finger(P>0.05).During a 6-month follow-up,the scores of each dimension and the total score of the Quality of Life Short Form(QOL-BREF)for the affected children showed significant improvement compared to before treatment,and the difference before and after treatment was statistically significant(P<0.05).Conclusions The use of the dorsal segmented polygonal skin flap for reconstruction of congenital syndactyly can effectively improve the function of the affected finger,enhance the aesthetic effect of plastic surgery,have fewer complications,and are safe and reliable;Cooperating with postoperative rehabilitation intervention can effectively improve the quality of life of pediatric patients.
