摘  要:目的评估计算机视觉技术引导下富血小板血浆(PRP)治疗对卒中后上肢复杂性区域疼痛综合征(CRPS)患者的疗效及其对上肢功能和日常生活能力的影响。方法为前瞻性、随机对照试验。2022年7月至2023年12月共纳入90例符合CRPS诊断标准的患者。参与者随机分配至对照组(n=45)和观察组(n=45)。对照组接受标准物理治疗,30 min/d,持续3周。观察组在接受相同的物理治疗基础上,额外每周进行1次计算机视觉系统引导下的PRP注射。治疗前后,采用简化Fugl-Meyer评分上肢部分(FMA-UE)、UCLA肩关节功能评分及视觉模拟评分(VAS)进行评估。结果治疗前,两组在FMA评分、UCLA评分及VAS评分方面差异无统计学意义(P>0.05)。治疗后,虽然FMA评分未见显著变化(P>0.05),但两组在UCLA评分和VAS评分上均有显著提高(P<0.05),且观察组的改善显著优于对照组(P<0.05)。结论计算机视觉引导下的PRP治疗能有效缓解卒中后CRPS的疼痛症状,并显著改善肩关节活动及功能。然而在提高患者上肢总体运动功能方面尚未观察到显著变化,仍需进一步研究以确认其潜在效果。Objective To evaluate the efficacy of computer vision-guided platelet-rich plasma(PRP)treatment in patients with post-stroke upper limb complex regional pain syndrome(CRPS)and assess its impact on upper limb function and daily living abilities.Methods This prospective,randomized controlled trial enrolled 90 patients meeting CRPS diagnostic criteria from July 2022 to December 2023.Participants were randomly assigned to a control group(n=45)or an observation group(n=45).The control group received standard physical therapy for 30 minutes daily over 3 weeks,while the observation group underwent the same physical therapy plus one weekly PRP injection guided by a computer vision system.Outcome assessments were performed using the simplified Fugl-Meyer Assessment for the Upper Extremity(FMA-UE),the UCLA Shoulder Function Score,and the Visual Analog Scale(VAS)for pain before and after treatment.Results Baseline comparisons revealed no significant differences between the groups in FMA-UE,UCLA,or VAS scores(P>0.05).Post-treatment,although FMA UE scores did not show significant changes(P>0.05),both groups demonstrated significant improvements in UCLA scores and VAS ratings(P<0.05).Notably,the observation group exhibited significantly greater improvements in shoulder function and pain relief compared to the control group(P<0.05).Conclusion Computer vision-guided PRP treatment effectively alleviates pain and enhances shoulder joint mobility and function in post-stroke CRPS patients.However,its impact on overall upper limb motor function remains inconclusive,necessitating further investigation to fully determine its therapeutic potential.
