犀角地黄汤联合雷公藤多苷片治疗湿疹的疗效与安全性研究

摘　　要：目的:探究犀角地黄汤联合雷公藤多苷片对湿疹患者的疗效,并探究其安全性。方法:选取2020年6月-2023年12月于笔者医院就诊的80例湿疹患者采用抽签法分为观察组(n=38)和对照组(n=42),对照组采用雷公藤多苷片治疗,观察组在对照组基础上加用犀角地黄汤,治疗8周。比较两组临床疗效和安全性。结果:治疗前两组湿疹面积及严重程度指数(Eczema area and severity index,EASI)评分、瘙痒程度评分和皮肤病生活质量指数量表(Dermatology life quality index,DLQI)各维度评分差异无统计学意义(P>0.05);治疗后,两组EASI评分、瘙痒程度评分和DLQI各维度评分均降低(P<0.05),观察组更低(P<0.05)。观察组总有效率94.74%高于对照组73.81%(P<0.05)。治疗前两组白介素(IL)-2、IL-4及肿瘤坏死因子(TNF)-α指标均差异无统计学意义(P<0.05);治疗后两组患者IL-2、IL-4及TNF-α水平均下降,且观察组更低(P<0.05)。治疗过程中,观察组出现口干、恶心、嗜睡各1例,对照组出现疲乏、头晕、口干各1例,两组均未出现严重不良反应,其不良反应发生率差异无统计学意义(χ^(2)=0.016,P=0.899)。结论:犀角地黄汤联合雷公藤多苷片能够提高湿疹患者疗效,改善其症状,提高生活质量,降低炎症因子的表达,同时保证了其安全性,值得在临床上推广。Objective To explore the efficacy and safety profiles of Xijiao Dihuang Decoction combined with tripterygium glycosides tablets in eczema.Methods Eighty eczema patients in the author's hospital from June 2020 to December 2023 were enrolled,and randomly assigned into two groups using lottery method.the control group(n=42)was given tripterygium glycosides tablets,based on this,observation group(n=38)added Xijiao Dihuang decoction.Treatment in both groups lasted for 8 weeks,then the outcome criteria including clinical efficacy and safety were compared between groups.Results The Eczema Area and Severity Index(EASI)score,itching score and the dimension score of Dermatology Life Quality Index(DLQI)at baseline yielded no statistical difference between groups(P>0.05).After treatment,a reduction was detected in EASI score,itching score and DLQI score in both groups,and the reduction was more notable in observation group than in control group(P<0.05).The overall efficacy rate was 94.74%in observation group,which was higher than 73.81%in control group(P<0.05).Baseline serum levels of interleukin-2(IL-2),IL-4 and tumor necrosis factor-alpha(TNF-α)demonstrated no statistical difference between groups(P>0.05).After treatment,there was a decrease in above three inflammation factors of both groups(P<0.05),and the decrease was more remarkable in observation group than in control group(P<0.05).During the course of treatment,one case each of dry mouth,nausea,and drowsiness reported in observation group,and one case each of fatigue,dizziness,and dry mouth occurred in control group,while no serious adverse reactions occurred in either group.No statistical difference was found in total adverse reaction rate between groups(χ^(2)=0.016,P=0.899).Conclusion Application of Xijiao Dihuang Decoction combined with tripterygium glycosides tablets in eczema can efficacy alleviate symptoms,improve quality of life,attenuate inflammation response,and has good safety profiles,which is worthy of promotion.
