摘 要:椎间盘退行性病变主要病理特征为髓核形态和分布异常,炎症反应、氧化应激反应、细胞凋亡、细胞自噬参与髓核退化,加重椎间盘进行性损伤。随着中医药在椎间盘退行性病变研究的深入,传统中草药中提取的有效成分在延缓椎间盘退行性病变的发展中发挥重要作用。骨碎补是中医治疗骨伤科的要药,骨碎补活性成分可促进髓核细胞增殖,促使椎间盘修复;抑制核因子-κB信号通路激活,调控丝裂原活化蛋白激酶信号通路,降低炎症反应,降低椎间盘炎性损伤;可抑制活性氧积累,降低氧化应激反应,减轻线粒体氧化损伤;可调控Wnt/β-catenin信号通路,调控Fas/Fas配体信号通路,激活磷脂酰肌醇3-激酶信号通路,增强细胞自噬,阻止NOD样受体家族蛋白3炎症小体活化,以抑制细胞凋亡,延缓椎间盘退变进程。The main pathological features of degenerative disc disease are abnormal morphology and distribution of the nucleus pulposus,inflammation response,oxidative stress response,cell apoptosis,and autophagy involved in nucleus pulposus degeneration,exacerbating progressive disc damage.With the deepening of traditional Chinese medicine(TCM)research on intervertebral disc degeneration,the effective ingredients extracted from traditional Chinese herbs play an important role in delaying the development of intervertebral disc degeneration.Drynariac rhizoma is an essential medicine in TCM for treating bone injuries.The active ingredients of Drynariac rhizoma can promote the proliferation of nucleus pulposus cells and facilitate intervertebral disc repair.The active ingredients of Drynariac rhizoma supplement inhibit the activation of nuclear factor kappa-B signaling pathway,regulate mitogen-activated protein kinase signaling pathway,reduce inflammatory response,and decrease intervertebral disc inflammatory injury.The active ingredients of Drynariac rhizoma can inhibit reactive oxygen species accumulation,reduce oxidative stress response,and alleviate mitochondrial oxidative damage.The active ingredients of Drynariac rhizoma can regulate the Wnt/β-catenin signaling pathway,regulate the Fas/FasL signaling pathway,activate the PI3K/Akt signaling pathway,enhance cellular autophagy,prevent NOD-like receptor thermal protein domain associated protein 3 inflammasome activation,inhibit cell apoptosis,and delay the process of intervertebral disc degeneration.
