摘　　要：结直肠癌(colorectal cancer,CRC)的形成与发展是一个多因素、多阶段的过程,涵盖遗传、表观遗传、肠道微环境和生活方式等多个维度。近年来,随着对肠道菌群研究的不断深入,CRC的发病机制已从单一的“炎—癌转化”学说扩展到包括慢性炎症、氧化应激以及肠道微生态失衡在内的多个层面,这提示肠道免疫在CRC发生发展的各个阶段均发挥着关键作用。现有研究证实,中药在调控肠道免疫和干预“炎—癌转化”上展现出独特优势。本文以肠道免疫为着眼点,探讨免疫应答异常激活、微生物失衡与CRC之间的关系,阐述中药通过修复固有屏障、调节免疫细胞,作用于肠道免疫修复,以促进机体抗肿瘤的潜在机制。此外,中药在增强免疫治疗的敏感性和疗效上也具备优势。对中药多成分、多靶点的干预特性的分析,可为CRC的临床治疗提供新的视角和思路。The formation and development of colorectal cancer(CRC)is a multifactorial and multistage process,covering multiple dimensions,including genetics,epigenetics,intestinal microenvironment and lifestyle.In recent years,with the deepening of research on intestinal flora,the pathogenesis of CRC has expanded from a single‘inflammation-cancer transformation’theory to multiple dimensions including chronic inflammation,oxidative stress and intestinal microecological imbalance,which suggests that the intestinal immune system plays a key role in the various stages of the development of CRC.Existing studies have demonstrated the unique advantages of traditional Chinese medicine in regulating intestinal immunity and intervening in the‘inflammation-cancer transition’.In this paper,we focus on intestinal immunity,explore the relationship between abnormal activation of the immune response,microbial imbalance and CRC,and elaborate the potential mechanism of Chinese medicine in promoting the body’s anti-tumor activity by repairing the intrinsic barrier and regulating the immune cells,as well as in repairing the intestinal immune system.In addition,Chinese medicines have advantages in enhancing the sensitivity and efficacy of immunotherapy.By analyzing the intervention characteristics of Chinese medicines with multiple components and targets,we can provide new perspectives and ideas for the clinical treatment of CRC.
