从“风痰瘀虚”分期治疗咳嗽变异性哮喘经验探析

摘　　要：咳嗽变异性哮喘(CVA)通常被认为是一种特殊类型哮喘,常表现为反复发作的刺激性咳嗽。近年来,随着社会环境、生活习惯的变化,该病发病率逐年升高,已引起社会大众广泛的重视。朱慧志教授临证治疗该病颇俱心得,提出“一本三因,分期诊治”的观点,认为其发病之本在于肺肾阳虚,外感风邪为诱因,伏痰和瘀血为关键病理因素,“风痰瘀虚”内外相合而发病。提出该病发作期以“风痰瘀”为核心,治以祛风通络,止咳化痰为主,兼通鼻窍,予经验方麻旋二虫饮加减;缓解期以“痰瘀虚”为核心,治以补肺温肾,化痰祛瘀,予经验方阳和平喘颗粒加减,临床收效颇丰。本文附治疗CVA医案1则,供临床参考。Cough variant asthma(CVA)is often considered a special type of asthma,often characterized by repeated attacks of irritating cough.In recent years,with the changes of social environment and living habits,the incidence rate of the disease is increasing year by year,which has attracted the attention of the public.Professor Zhu Huizhi has a lot of experience in the treatment of this disease,puts forward the viewpoint of“one root,three causes,diagnosis and treatment by stages”.She thinks that the origin of this disease lies in yang deficiency of lung and kidney,exogenous wind evil is the inducement,phlegm and blood stasis are the key pathological factors,and“wind-phlegm-stasis-deficiency”are combined internally and externally.It is suggested that the core of this disease is“wind-phlegm-stasis”in attack period,the main treatment is to expel wind and dredge collaterals,relieve cough and dissipate phlegm,also to clear nasal orifices,the empirical formula Maxuan Erchong Yin is modified;the core of this disease is“phlegm-stasis deficiency”in remission period,and the main treatment is to tonify lung and worm kidney,dissipate phlegm and remove blood stasis,the empirical formula Yanghe Pingchuan Granules is modified.Clinical effect is quite good.A case of CVA is attached for clinical reference.
