度普利尤单抗治疗特应性皮炎诱发泛发性脓疱型银屑病1例

摘　　要：本文报告1例特应性皮炎患者在使用度普利尤单抗治疗4周后,全身出现红斑脓疱伴脱屑,经诊断为泛发性脓疱型银屑病。停用度普利尤单抗后,改用白细胞介素-17A抑制剂依奇珠单抗治疗,1周后皮损显著消退。本病例提示,度普利尤单抗可能诱发银屑病样皮损,提示特应性皮炎与银屑病虽临床表现不同,但可能存在免疫学关联。近年来,针对不同免疫靶点的生物制剂在临床中广泛应用,但其潜在风险仍需警惕。临床医师应加强病情监测,优化治疗方案,未来需进一步探索其作用机制,以提供更安全、有效的治疗选择。This article reports a case of atopic dermatitis in which the patient developed generalized pustular psoriasis with erythema,pustules,and desquamation over the entire body after four weeks of treatment with Dupilumab.After discontinuation of Dupilumab,treatment with interleukin-17A inhibitor Ixekizumab was changed,and the lesions subsided significantly after one week.This case suggests that Dupilumab may induce psoriasis-like lesions,indicating that although atopic dermatitis and psoriasis have distinct clinical manifestations,they may share underlying immunological connections.In recent years,biologics targeting different immune pathways have been widely used in clinical practice,but their potential risks still require vigilance.Clinicians should strengthen disease monitoring,optimize treatment plans,and further explore the mechanisms of action to provide safer and more effective treatment options.
